The family health assessment identified different social determinants of health (SDOH). SDOH shapes different conditions ranging from age, work, growth, physical environment, education, and economic status (Friedman & Banegas, 2018). The identified elements are simply a group of SDOHs; however, more elements should be considered while working with a specific family. For instance, the family under study had specific issues which socially determined their health. It would result in positive engagement, health restoration, and effective social support networks whenever considered effectively. The information obtained from the family health assessment is relevant since it determines the level of access to health care through considering family structures and theories. The paper probes SDOHs identified in the family health assessment and effective action plan to address different elements of the determinants.
Social Determinants of Health
The critical social determinant of health affecting the family health status is low socio-economic income. The family can be classified as a low-income family based on the finding. It is a type of determinant that jeopardizes the health of the family since it determines resources access and acquisition. The impact that the SDOH has on the family is that it limits different family members from successfully accessing healthcare. It is prevalent in the family due to the unstable employment of the parents. Therefore, the family cannot generate adequate resources in accessing equitable health care. The second SDOH noted in the family is the physical environment. The family has been exposed to a poor physical environment ever since. The results of the interview show that the family is not physically active. Physical activity is essential in health since it reduces possible elements such as high-fat accumulation and other factors (Kaakinen et al., 2018). Physical inactivity impacts the family by exposing it to different diseases and heart problems. It is prevalent in the family due to poor action plans necessary for successful exercises. Therefore, it is justifiable that the identified SDOHs are critical since they adversely impact the family health status and a reliable action plan is necessary to restore the family health.
Age-Appropriate Screenings
Based on the information obtained from the family health assessment, there are specific age-appropriate screenings that each family member must undertake. For instance, the family is composed of three members; the parent's and two siblings' health must be secure. Both the parents should undergo a cholesterol check. It is a screening where the cholesterol level is identified and analyzed whether it jeopardizes the overall health. The rationale behind such screening is due to the parents' ages, which are 38 and 30. Universally, cholesterol check occurs between 18 and 39 years; therefore, they are viable for screening (Friedman & Banegas, 2018).
