Panel Management. Relationship between Diabetes and Obesity in the US

Essay Instructions:

Considering the implications of the data in Tables 2 and 3 below about the condition prevalence and risks, describe in a 3-4 page paper how a practice of 100 physicians—perhaps configured as an ACO—would manage the risks and conditions of 14,500 diabetics and 160,000 people who are overweight or obese. Also, describe how the physicians would find out which of these patients are at risk for diabetes but don’t know it yet. DATA TABLES ARE IN THE ATTACHMENT!

Relationship between Diabetes and Obesity in the US
Introduction
Diabetes is one of the chronic illnesses that are most prevalent and expensive to treat(Centers for Disease Control and Prevention (CDC), 2019). An approximate of 23.1 million individuals are diagnosed with diabetes in the United States, the treatment resulting in an annual cost of more than $245 billion. Researchers have shown that diabetes and obesity are linked together as risk factors for one another; Greater than 90% of diabetes patients are obese or overweight (Bramante, Lee & Gudzune, 2017). Prevalence data is from recent US consensus. Data from table 2 and 3 indicate a very different prevalence for both diabetes and weight management patients (Appendix A). Pre-diabetes is often left undetected due to the nature of poorly established methods of identifying early markers, hence it’s progression into diabetes is often inevitable. US is the top 50 list of countries for both prevalence of diabetes and obesity (Bhupathiraju & Hu, 2016). The increase in the prevalence has resulted in need for urgent care and attention towards factors which contribute towards the diseases along with early diagnosis to prevent progression of the two.
Risk and Conditions
Diabetes mellitus (DM) is an alarming global public health crisis. The number of diabetes mellitus (DM) patients globally is growing quickly which is supported by data from online consensus comparison with the figures in table 2 (Appendix A). A rise in body fat usually results in an enhanced risk of acquiring metabolic diseases such as diabetes type 2 (SebehAlhazmi et al., 2017). The connection between body weight and metabolic abnormalities is vital towards gaining a better knowledge of the fundamental mechanisms of pathophysiology.
Diabetes
Strong proof for interventions have shown to avoid or delay type 2 diabetes among high-risk patients specifically people with pre-diabetes, organized lifestyle (exercising more often and consuming more fiber but less complete calories, fats and sugar-sweetened drinks) decrease the occurrence of type 2 diabetes through 30% to 60% (Siegel et al., 2018). If the 100 physicians treat each case on individual bases the number is sure to decrease significantly from the 160,000 (Appendix A). Age, sex, race and ethnicity and genetics factors are among the few risk factors that impact the prevalence to diabetes in the US.
Obesity
More than 1/3 of individuals in the United States have or are at risk of obesity. Obesity is affiliated with a variety of risk factors, including diabetes, cardiovascular disease, obstructive sleep apnea, and cancer (Fruh, 2017). Comparing the figures given in table 2 and 3 we can make an assumption that 64% patients require weight management (Appendix A). By evaluating the risks in individuals, creating reasonable objectives for weight loss, and providing patients with the information and therapy instruments, we can assist in accomplishing weight loss. This can further be followed by structured lifestyle changes to preserve the results gained.
