A care coordination plan is a deliberate approach to organizing healthcare services and activities and exchanging relevant information about specific health concerns. For safer and more effective care outcomes, patients' needs or preferences are determined in advance and communicated to stakeholders at the right time. Approaches applied to attain such objectives include integrating teamwork, managing medications, utilizing health information technology, and ensuring patient-centered practices (Maresova et al., 2018). In this paper, a health concern identified for preliminary care coordination is a cognitive impairment of Alzheimer's Disease.
A Health Concern
As society grows old, age-related illnesses increase in prominence as public health concerns. Cognitive disorders like Alzheimer's Disease (AD) are imperative and most common in the aging population. The prevalence of AD in the world is about 50 million people affected, and it is projected to rise by double in 2050 (Zhang et al., 2021). AD is a progressive condition affecting memory or vital mental functions, and though presumed to occur with age as the greatest risk factor, it is not a normal part of aging. In a society like the United States (US), AD is the leading cause of death, with those affected living an average life of fewer years after symptom onset (Armstrong, 2019). However, survival depends on care interventions, age, and other complications.
Physical needs; - Since Alzheimer's is a progressive form of dementia, it slowly affects cognition and the ability to engage in physical activities. With that, patients rely on daily care and long-term support use. As the condition progress, a person will require assistance with daily living activities (ADLs) such as grooming, eating, and movements (Maresova et al., 2018).To remain physically active, an AD patient needs help to keep muscles, joints, and the heart in good condition. At the same time, one require adequate sleep and rest. In most case, stability is important to these patients because moving from one place to another or relocating from one neighborhood to another might cause confusion and disorientation. Lastly, these patients are always encouraged to exercise for 30 to 60 minutes weekly. This plays a vital role in promoting thinking skills like critical thinking.
Psychosocial needs include a combination of emotional, spiritual, behavioral, and mental concerns. Psychosocial interventions are necessary and productive for agitation since even if a person is cognitively impaired, he remains responsive to interact with people. Agitated AD patients respond to others during personal conversations or react to films, images, or music familiar to them. As the condition progresses, the patient's social role in a family change, so they may forfeit autonomy when someone else takes over their responsibilities. Most families choose institutional placement for AD patients when care gets overwhelming. Another underlying assumption is that institutionalization would relieve the physical and psychosocial burden.
