Describe the various duties a nurse might perform when working in transitional care.
In its simple form, transitional care is defined as the safe movement of a patient from one hospital or care setting to the other. While the patient is discharged or move to another health care center, it becomes necessary for the nurse to coordinate and cooperate throughout the process. Ideally, they can prepare the medical report of the patient and help him or her in clearing the dues. The bedside nurse may be better able to understand the requirements of the patient, so they should travel with him to explain things to the representative of the new care setting. Similarly, the senior nurse may accompany with him all medical reports, receipts of medications and details of the equipment used to treat the patient. Nurses can easily create a transitional care plan for the patient by compiling all information, and then share the same plan with the new care team. One of the most important things is communication during the handoff process.
