The capstone project is a combination of previous course work to help you integrate the knowledge and learnings acquired in the MHSA program.
You will have named and identified which type of health care organization/facility you will construct through substantial research. You will have created a comprehensive project that can be used as a portfolio entry.

* Rationalize the organizational theory of your selected organization/facility. In this section, identify how diversity and cultural competence will be maintained within the organization to meet the dynamics of the population identified.
* Identify the organizational behavior methods that will be used in your organization/facility.

Mission
To provide personalized, comprehensive care and comfort to patients and families facing a life-limiting illness, disease, or condition through the end of life.
Vision                                              
To be the Hospice and palliative facility representing the best option for patients at the end of life and families requiring hospice and palliative care services.
Visitors are welcome 24 hours a day, depending on a patient’s health condition. As part of caregiver support, family and friends are included in care where professional counselors and other staff members facilitate care delivery.
2. Data Examination
* External environment and demographics 
The hospice is located in a quiet area that is easily accessible by visitors, which is helpful when treating and managing patients with a life-limiting illness. There is an emphasis on providing comfort to the patient and family in a dignified manner. According to CityData (2021), Miami had a population of 467,963 in 2019 and 100% population is urban with males being 231,737 (49.5%) and females, 236,226 (50.5%). The median resident age was 40.7 years low than Florida’s at 42.4 years.
The median household income in 2019 was $42,966 and lower than the state average of $59,227. The racial demographics of those living in poverty indicate that 8.3% of the residents are White Non-Hispanic residents compared to 30.4% Black people. Hispanic or Latinos are 20.9%, while 14.6% for American Indians, 38.2% for Native Hawaiian and other Pacific Islander, 26.6% for other races, and 18.9% are two or more races (City Data, 2021). 
Medical Conditions 
 Data on Miami is mostly aggregated in the Miami-Dade county statistics. Still, there are differences across the county because of the demographics and socio-economic conditions in the urban county. Among the common medical conditions in the city of Miami are the conditions associated with obesity and being overweight, including diabetes and hypertension. Miami-Dade County, FL has a higher rate of preventable hospital admissions among Medicare beneficiaries than the state and US average.
Catering to the medical conditions
A multifaceted approach to creating a supportive environment is necessary to promote healthy living. Obese and overweight patients tend to get suboptimal end-of-life care. Care delivery affects health outcomes and people’s willingness to seek hospice services (Honinx et al., 2021). The holistic approach is relevant to meeting each individual’s unique health needs by focusing on wellness and not merely conditions or illnesses that necessitate health care service. Even end-of-life care requires optimal care and the holistic model emphasizes considering the physical, psychosocial, emotional, and spiritual needs.
