A 5-year-old male is brought to the primary care clinic by his mother with a chief complaint of bilateral ear pain for the last three days. The mother states that the child has been crying frequently due to the pain. Ibuprofen has provided minimal relief. This morning, the child refused breakfast and appeared to be “getting worse.”
Vital signs at the clinic reveal HR 110 bpm, 28 respiratory rate, and tympanic temperature of 103.2 degrees F. The mother reports no known allergies. The child has not been on antibiotics for the last year. The child does not have history of OM. The child is otherwise healthy without any other known health problems.

Search Strategy, Original/Revised Guideline, its Development and Date of Publication
Acute otitis media is one of the most common acute infections occurs in children, who are from 6 months to 12 years. The American Academy of Pediatrics and the American Academy of family physicians together developed the original guidelines of evidence-based practice for the treatment of acute otitis media in the year 2004. These guidelines have been revised by the American Academy of Otolaryngology-Head and Neck Surgery Foundation in the year 2016 (Rosenfeld et al., 2016). The search strategy is to find the latest guidelines by which the controlled trials could be conducted. The guidelines were originally published on 1st February 2016 by different scholars, including Richard M. Rosenfeld, Jennifer J. Shin, Seth R. Schwartz, Robyn Coggins, Lisa Gagnon, Jesse M. Hackell, David Hoelting, Lisa L. Hunter, Ann W. Kummer, Spencer C. Payne, Dennis S. Poe, Maria Veling, Peter M. Vila, Sandra A. Walsh, and Maureen D. Corrigan. These guidelines are helpful in diagnosing the case study of a child who has acute otitis media.
Systematic Review of Current Best Evidence
           A recent systematic review has been done to analyze and evaluate the effectiveness of guidelines with regards to the best evidence-based practice. A study uncovers that annually there have been 709 million reported cases of acute otitis media and half of the reported cases are in the age group of under 5 years (Daniel, Qureishi, Lee, Belfield & Birchall, 2014). It reveals that almost 80% of children get relief from otitis media within 2-14 days with the treatment suggested in the guidelines (Daniel, Qureishi, Lee, Belfield & Birchall, 2014). Moreover, the guidelines suggest that the treatment lies in the use of antibiotics for mild and severe unilateral or bilateral acute otitis media.
Conflict of Interest Management and Quality of Evidence
The guidelines co-developed by the American Academy of Pediatrics and American Academy of family physicians were further updated by the American academy of otolaryngology-head and neck surgery foundation to improve its effectiveness. The quality of evidence is analyzed by the confidence shown by experts on the research outcomes. Moreover, it can be evaluated through the fact that the clinical practice guidelines were formulated based on 20 systematic reviews and 49 controlled trials to track and rectify any flaws. Thus, in the revised guidelines, special emphasis is made on educating the patient in order to avoid recurrence of otitis media to some extent. Moreover, new recommendations for the management of otitis media is given to a child which is subject to the new-born hearing examination (Rosenfeld et al., 2016). Thus, the updates in the guidelines have further improved the quality of care given to manage acute otitis media.
