A communication procedure between community pharmacists and primary care professionals resolves medication errors and other administrative issues. In this study, Oñatibia-Astibia et al. (2022) investigated whether a communication intervention between primary health professionals (PHCPs) and community pharmacists (CPs) could solve medical errors. With 49 community pharmacies participating in the study, the authors first carried out a six-month pilot study followed by a one-year principal study, with email being the primary communication method between CPs and PHCPs. Throughout the course of the study, 1179 medical errors and administrative issues were identified, with 90% of the challenges addressed within 24 hours. The findings of the study highlight the significance of enhanced communication between PHCPs and CPs in addressing medical errors. The main takeaway from the article is that with an adequate communication system between doctors and pharmacists, medical errors can be reduced to a significant extent. The article has valuable information that support the value of communication in preventing medical errors.

Soares Santos et al. (2021) conducted an integrative literature review to identify publications about medical errors in primary care. The authors analyzed 17 studies to determine causes of and solutions to incidents of medical errors. The main causes of medical errors include incorrect doses, prescriptions with incorrect drugs, drug allergies, and drug interactions. One of the main solutions proposed by the review is the utilization of information technology tools, such as electronic prescriptions, to help health professionals improve the safety and quality of prescriptions.
