A near miss in the field of medicine refers to a situation where harm that was to occur to a patient is prevented by timely action by the health care provider, family, the patient, or in some cases, due to good luck (Espin, Carter, Janes, & McAllister, 2019). Near misses are sometimes referred to as close calls or good catches. In a culture where safety is always practiced, near misses are taken to be free lessons for all the people around. Near misses can sometimes occur several times before the actual expected harm occurs. Many reported several near misses have preceded avoidable deaths. Organizations that are considered to be of high reliability take near misses to be a learning lesson and a chance for improvement. The organizations always make assumptions; ask themselves how the next patient may be affected, and therefore value the knowledge and the input they get to make improvements (Raymond, Godfrey, Medves, & Ross-White, 2017). However, low reliability organizations become falsely reassured that no harm can occur and that their system is safe and this increases the chances of the actual harm occurring.
Near misses are important because they represent error prone situations and error traps that wait to face other patients and therefore be corrected immediately. They are also important because they pose less anxiety and less blames on other people as they are still no victims who have been harmed. Most big hospitals have offices where their patients can report near misses and they always encourage their staff to report such cases (Pawlicki, Coffey, & Milosevic, 2017). Reporting is important because through that ; the risk that may have faced the patients is reduced before it can actually face them. It triggers improvements in the weak points that may have been noted in the work place and helps alert other healthcare providers in gaps that may need trainees. It also contributes to harm reduction strategies before an event that may result in harm occurs.
A database in medicine refers to a group of information that has been stored for other people to access it and retrieve the information when needed. For the research dealing with near-miss medication, there are several databases that can be used but the most commonly used type is the OLTP (Milosevic, Angers, Liszewski, Drodge, Marchand, Bissonnette, & Louie, 2016). OLTP stands for online transaction processing type of database. The healthcare databases will form the base for running several transactions found in the industry. There are many applications found in OLTP and these include EHRS, the systems that show patients are satisfied, and patient identification.
