Liver cirrhosis is still one of the most delicate factor in the determination of a successful surgical operations that deal with the cardiopulmonary system. There seems to be a greater risk in performing such operations on patents with liver cirrhosis as a preexisting condition. Research into th same has not been quite extensive as there seems to be quite a small sample for study since the number of patients with both conditions is relatively small. The Model End-stage Liver Disease (MELD) score has often been used in determining the scenarios involving short term survival abilities for patients with both conditions. It can therefore play an important role in the prediction of the outcomes, be they early or late, for patients with liver cirrhosis that undergo surgical procedures.
Summary of the article
The article “Impact of MELD score and cardiopulmonary bypass duration on post-operative hypoxic hepatitis in patients with liver cirrhosis undergoing open heart surgery” was a culmination of a research done by a team of four researchers ; Chen,Lin,Yu and Chou. The team worked based on the fact that there was too much uncertainty when it came to the determination of the final results of surgical procedures on patients who had liver cirrhosis. The research uses post-operative hypoxic hepatitis (POHH) as the primary end point and concludes that surgery should be avoided in liver cirrhosis patients because it increases their mortality rate.
The result of the research was based on 61 patients (above 18 years) who underwent open heart surgery from 2007 to 2017. The seriousness of LC in the participants was classified by MELD score and Child–Pugh classification. As a result, all the 61 participants were diagnosed with LC after an extensive examination of laboratory data, ultrasound, and various clinical features. Other common features among the participants are the presence of nonspecific biological abnormalities, history of ascites, elevated glutamate oxaloacetate transaminase, and esophageal varices.

The research is a retrospective study because all the participants have various features in common but differ with regards to the MELD score. The comparison of the results was based on participants with a MELD score that ranges from 5 to 20 and those with a MELD score of above 20.
