HPI:
Nasal congestion, clear rhinorrhea, and frequent sneezing that began about one week ago. One week ago began experiencing itchy and watery eyes in addition to the nasal congestion and rhinorrhea. Reports facial pressure around cheek bones and eyes. Moderate amount of clear drainage from nose with persistent congestion. Reports sore throat upon awakening in the mornings 2-3 days a week that usually resolves on own by mid-morning. Symptoms tend to be worse in the early mornings and in the evenings. Denies facial pain, nasal pruritus, ear pain, ear drainage or popping. Denies fever, night sweats, headaches, changes in vision, cough, or change in appetite. Patient reports symptoms initially began after going out one night with friends to a restaurant for dinner. Reports lots of time spent outdoors lately at soccer practice. Reports having a new friend who is a smoker; congestion seems worse after being around his smoking friend. Reports having seasonal allergies as a child for which he took nothuibg” but didn’t have any problems as a teenager or since he has been away at college until now. Has been taking Mucinex-D which helps for a few hours but the symptoms always return. Has been taking Benadryl at night to help him sleep, but it sometimes leaves her feeling groggy in his morning classes.
For this assignment, the student will create a patient education pamphlet or flyer that you will use to educate your patient after identifying a nursing diagnosis when conducting your physical assessment. The tool should be uploaded into this assignment area with your video of you teaching the patient using your created pamphlet or flyer.Suggestions for the tool used should address the needed patient education information and must be patient-centered and evidence based.
