The Golden Age Hospital (GAH) values the health of the geriatric population. The mission of GAH is to provide exceptional care and services to our patients, team members, and partners. Evidence-based medicine, provided by the very best team of professionals, delivers gold standard health care for the health equity of our senior population. Since the Golden Age Hospital serves patients of all backgrounds and demographics, the proposal will include departments focused on maintaining compliance by remaining updated on federal, state, and local regulations; to qualify as a 501(c)(3) organization (James, 2016). Completing the Community Health Needs Assessment will ensure resources are utilized appropriately, and guidelines are met to benefit from special tax deductions. Additionally, personnel working in GAH are highly-trained thus earning the position as a magnet hospital.
Furthermore, the functionality of an electronic health record system with provider and patient portal access ensures accurate information is available when needed. GAH will provide an organizational chart to identify individual roles and job functions of those overseeing each department, along with fostering the interdisciplinary care coordination approach between GAH and community partners. There is a consideration for an alliance with other hospitals and providers as part of an Accountable Care Organization since Medicare and Medicaid will fund most services.
