In the article “Clinical information systems end user satisfaction: The expectations and needs congruencies effects”, it discusses the attitudes of information system users and how their continuance of use intentions are associated with their satisfaction of the systems they use (Karimi, Poo, and Tan, May 2014). It is important for information system designers to understand the needs of the users in order to properly serve their needs within a system. When information systems are being designed for the healthcare industry, it is important to have a grasp on what aspects are necessary. This can be done by involving end-users in the development of their own tools which can help to create applications that are enhanced to meet their needs as well as protect the organization from tools that are unauthorized which can cause harm to their system protection (Elizabeth O’Dowd, August 2018).
 
It is important for developers to understand what is needed from CIS as this affects the user’s communication and interactions with their healthcare team and more importantly their communication with their patients. Healthcare professionals are more commonly using CIS in their offices to care for their patients and store their patients’ electronic medical records. It is important that systems work effectively in order to properly care for and treat their patients. One of the most important things the developers can do is shadow what needs there are for an organization’s CIS and ensure those needs can be met through implications within the system.
 
Personally, I do not think there would be much success without end-user input in the system design process. The only way this could work is if it were a standardized system that has been tested and proven effective with other healthcare organizations. In order to know that a CIS is going to work properly and fulfill the needs of the users, you have to first know their needs and develop the system to meet those expectations. Users want systems that work effectively, error free, and are easy to use. These are all things that are important for developers to keep in mind regardless of if the user is in fact a part of the design process or not.

The United States is one of the few countries without a single payer healthcare system in place.  Many of the other countries that have a governmental funded healthcare system experience a great deal of benefits; however, there are disadvantages as well.  Though there are several groups of stakeholders involved, some present as more prominent in the news or society where benefits are concerned.  They seem to be those who do not currently posses healthcare insurance or lack access to healthcare along with the insurance companies as well as big pharma.  From a stakeholder standpoint, the reality is, private insurance companies stand to loose a great deal of money as does big pharma and the American people will see a significant payroll tax.  Businesses could also see new taxation.  There is an idea that a single payer healthcare system would save money, bottom line.  At the Mercatus Center of George Mason University in 2018, Charles Blahous, published a study that showed a single payer system would likely not save money in the long run.  His study showed that over the course of a ten year period, 32.6 trillion dollars would be added to federal budget costs.  He argues the idea that giving healthcare to the public for free would cause an inevitable increase in usage.  This could be due in part to the increased number of people that subsequently begin to utilize the free benefits who may not have been previously privileged to as much; however, a portion could also come from over usage by those who may not have accessed their benefits in their entirety in past times. 
     The idea that it is fair for the "haves" to be required to pay additional and/or corporate taxes to provide medical services for the "have nots"  is not necessarily accurate.  The word fair is subjective depending on how it is being used and whom is being asked.  If we ask those who are wealthier and have worked their entire lives to reach a point of plenty, having come from meager beginnings, quite honestly they may have a stronger argument as to why it isn't fair they be taxed on a much higher level.  There are very few who are more financially strong than most that will say, "yes it is fair".  While it is an absolute truth that the less financially fortunate continue to need healthcare as do wealthier people, it may be yet wise to remember that individuals who are blessed with wealth are yet people too.  They are important as well.  At times individuals who struggle financially may assume those who do not are simply greedy and if they would just see reason and submit to the single payer healthcare system, the United States could be better for it.  The most wise approach lies in the admission that regardless of our financial position, remembering we are all people with needs and ideals is key.  If the wealthiest people in our country were taxed at a greater rate, without taking so much as to negate their efforts to reach such a place in life; it would be alright.  It wouldn't necessarily be fair to them; however, if it was done as modestly as possible it would be a less difficult pill to swallow.  On the same token, workers should not have to give up their current employer insurance.  They should be given a choice.  Having a choice would give many peace of mind that they are allowed to determine whether or not a medical procedure is necessary for them or a loved one.  Many do not want big government to decide what is necessary in their lives.  Many Americans want the opportunity to choose for themselves.

The United States is registering a continuous rise in health care costs as compared to inflation, which drives the need for health care reforms. By the onset of 2016, the U.S. spent 17.9 % of the GDP on physician services, hospital care, and prescription drugs equivalent to 20%, 32% and 10% respectively (Hartman et al., 2018). Essentially, the country lacks a uniform health care system, and the main hindrance to achieving an extension of healthcare services to every person is the required funds from stakeholders. As such, I believe that by having the “the haves” pay for additional corporate or personal taxes in the provision of medical services for the “have nots” is a fair approach.
Workers should not have to give up their employer-sponsored private-pay insurance to move to a single-payer system. Notably, depending on the single-payer system is likely to be disadvantageous as a huge amount of the population often relies on one health insurance coverage resulting to persistent inequities in the access of care (Dickman et al., 2017). Also, the single-payer system is prone to system failures with the higher population it serves, thus, resulting in ease of fraud by breaking the bookkeeping. As such, employees should be entitled to private-pay employer-sponsored insurance coverage or be allowed a choice in between to allow equal access to effective health care services. In addition to obtaining adequate care services, allowing the choice of health care coverage will also encourage equity by allowing those that do not have insurance to get benefit from either and reduce twinning of health care insurance coverage.

I like your elaboration of the end-user involvement on the design of the programs assisting healthcare professionals in their management and application of computer information systems (Alotaibi & Federico, 2017). It gives adequate plans to ensure they are appropriately applied. However, with the innovations of more programs not only for healthcare but to be used in other organizations, do not you think external developers can also be of help? I recommend that the IT department leaves room for other developers who could have more experience to assist in providing the correct design, test, and launch the use of such programs in healthcare. Finally, would there be challenges with my thought?
