The Challenger/Columbia disaster case represents phenomenal managerial failures that led to the loss of lives and property. The 1986 Challenger disaster was attributed to various reasons such as uncoordinated decision making bodies in different geographical locations, failure to implement expert recommendations such as replacement of O-rings whose reliability was questioned and overlooking critical expert warnings and decisions such as warning to change the launching date due unusually cold conditions (Palmer, Buchanan & Dunford, 2008; p.376). The Columbia disaster was attributed to failure by the management to uphold the post-Challenger changes through failing to implement expert recommendations, flawed communication between departments and employees, making flawed analysis and overall showing ineffective leadership (Palmer, Buchanan & Dunford, 2008; p.379).
