Part 1.
In the last twelve months, I have been working with Jill, who has not been exhibiting any progress from the time she began her psychotherapy sessions. When she started her treatment, she mentioned experiencing much anxiety and depression. She would also get angry so often hence needed to overcome her depression. Despite the therapy sessions we had with Jill these couple of months, she sat in front of me tired and looking depressed every day. She had mentioned that since her parent's death in a fatal road accident, things have become chaotic. She has since had to take care of her two younger siblings, who equally have not gotten over their parents' death.
I began Interpersonal psychotherapy (IPT) with Jill hoping this would help her recover from the tragic event that occurred to her family. I believed this would help her cope with the feelings of losing her loved ones all at once. I thought the IPT would help Jill enhance her social functioning and, as a result, reduce her anxiety. At least some progress from the strategies that I have shared with her would have improved her condition. However, since the time we began our sessions, Jill is still very distressed. She says her responsibilities are the things that keep weighing her down.
Usually, the IPT helps patients manage their grief and is the most preferred for those who have lost a very close loved one. This treatment often brings out the best in patients who might have experienced this loss recently. IPT is known for treating severe depressive disorder and would also resolve maladaptive thoughts because of interpersonal relationships. However, Jill seems not to respond well to this treatment as she still breaks down during sessions, and other times she would send her apologies for not being able to make it because of other commitments.
Part 2:
The individual therapy sessions I have had with Jill include continuous assessments and frequent interviews whenever she is around. Sometimes I would give her assignments to do while at home on the days she announced her absenteeism. The first IPT session involved assessing her depressive symptoms and history with relatives. I realized that several changes occurred since her parents' death, which has affected her relationship with the siblings. After conducting this analysis, I worked with Jill to develop specific treatment strategies for dealing with her parent's demise. These strategies could change in her attitude, yet very little or close to nothing at all have occurred.
