Introduction
Upon admission, Mr. Williams is a 79-year-old African American widower that is currently residing in a foster care home. He has lived in the home care for at least four years since he lost his wife. He was a former minister during her professional career. His medical history includes a long-term diabetes type II, high blood pressure, and benign prostatic hypertrophy. The home care provider has requested a home visit to evaluate Mr. Williams. As based on the Geriatric Resource, this is to apply the foundations of improving geriatric care as part of the acute care for the elderly through the niche GRN model. The mission is to conduct a comprehensive geriatric care to the patient that is supported by the application of interdisciplinary models of care for the elderly through home care within the elderly homes.
As part of the model, the application of community-based health care practice seeks to alleviate the signs and symptoms of the patient's condition while they are confined in the hospital setting. The nurse will be collaborating with other health care providers, peer consultants, quality change agent, and educators. The evidence based practice promotes an interdisciplinary team through channeling and communication. Another model is the acute care model, in which the clinician provides a comprehensive physical assessment of the patient as well as taking their past and present medical illness to understand the current health care condition of the patient. Taking initial vital signs reveals the present condition of the patient such as taking the rates from their respiration, pulse, temperature, and blood pressure. In addition, engaging in efficient rapport helps to introduce the health care provider to the patient to establish trust as well as cooperation with the implementation of the geriatric care procedures (Touhy & Jett, 2014).
