Assessment Description You will utilize your change model for this assignment. Review the feedback submitted by your instructor on your previous change model assignment. Make any changes or modifications necessary for the submission of this assignment.
Evaluate the performance of your organization or department. Identify an area that would significantly benefit from initiating a change. Write a paper (1,500-1,750 words) in which you describe the particular area you propose to address through a change initiative. Include the following for your company:
Discuss the issues in this area and the current outcomes as a result of the issues.
Describe the external and/or internal driving forces, contributing issues, and the people affected.
Evaluate the stakeholders involved and discuss how they will be affected by your change initiative.
Clarify your role and responsibility as a change leader. Discuss the leadership theory (or theories) you will use to guide the change process.
Discuss the change agents you need to recruit in order to successfully implement your change. Describe the roles of these change agents.
Utilize your change model to develop strategies: (a) Explain the relevance of this model to your organization; and (b) Present the strategic aspects using your model. Be sure to clearly define the purpose of each aspect, the people involved, and the actions that need to be taken.
Identify, or predict, the potential barriers to change. Discuss possible ways to overcome these obstacles, including methods for dealing with emerging or unforeseen circumstances that could impede implementation.
Describe the evaluation methods you will use to determine the level of success of your change initiative. Discuss what metrics or measureable determinates you will use.
Propose strategies to anchor change or support continuous change.
Establish how your change plan supports the organizational mission/goal, genuinely addresses stakeholder concerns, and will serve as an equitable contribution for the community or society overall.

Organizational change is an essential ingredient to an organization. It is responsible for improving organizations’ performance by introducing new ways of actualizing processes and actions. However, to be successful, change in an organization must be aligned with stakeholders’ needs, and there must be a need for change. This paper details how Consulate Health Care can adopt a change of improving nurses’ workload, especially in its nursing sector. Patients are the primary beneficiary of this change by being guaranteed safety. The hospital and nurses will also benefit through minimization of cases of burnout among nurses and increased patients’ admission in the part of the hospital. Kotter’s 8-Step model guides the proposed change.
The Problem at Hand
The Consulate Health Care is a premier national provider of senior medical services. It specializes in post-acute care. Its primary services include Alzheimer’s and Dementia care, long-term care, comprehensive rehab, assisted and independent living (Consulate Health Care, 2021). It strives to ensure that these services are offered on a 24 hours basis. This light means that the hospital’s nurses work around the clock to provide quality services that are administered promptly. But this mode of working raises several questions. In particular, the question about giving nurses enough time to balance between work and personal engagements. It may also be likely that working 24 hours predisposes patients to safety concerns because some nurses can be tired and are poised to offer sub-standardized care, which has a severe effect on both the hospital and patients. The patients will be affected by experiencing prolonged pains, and some may succumb.
From a hospital perspective, lack of enough nurse workforces may lead to mass patient walkouts, leading hospitals to record losses. Lack of enough nurses is a longstanding issue in the medical field. Li-Yin, Hsiu-Hui, & Yann-Fen (2019) concede that nurses increased hours of working correlates with risks of indices such as decubitus, tube slippage, and urinary tract infections increase. This revelation confirms that proper nurse staffing is an issue of global concern and not only felt in Consulate Health Care. Therefore, the hospital must institute approaches to mitigate the problem to guarantee patients’ safety and improve nurses’ job productivity.
Driving Forces (External and Internal), Contributing Issues, and the People Affected
External and internal factors advance the change. The external factors range from acknowledging the shortage of nurses to the need to offer individualized care. Tamata et al. (2021) affirm this assertion by noting that it is estimated by 2025, there will be a shortage of 7.2 million health workers responsible for delivering healthcare services. Compounding this problem is the fact that there is an increase in the aging population, with an estimated increase of 73 percent from 71 percent in 2019 among Americans aged 65 years (Haddad et al., 2020).
