Healthcare Delivery Models and Nursing Practice
The purpose of this paper is to examine the healthcare delivery models and the nursing practice context. This report evaluates the reforms within the American healthcare system by focusing on the affordable care act and its related reforms, the impact of pay-for-performance programs and quality measures on patient outcomes, professional nursing management and leadership roles within the emergent federal law implementation situation, and the emerging trends within the healthcare sector.
Federal Healthcare Law Reform
The Affordable Care Act (ACA) is a healthcare law introduced in 2010 to expand health insurance. Since its passage, the law has increased the population of Americans with health coverage to over 20 million and brought uninsured rates to an outstanding low. The primary components of ACA tailored to scale up coverage include the creation of medical insurance marketplaces along with cost-sharing subsidies and premium, eliminating coverage restrictions due to pre-existing medical conditions, allowing persons up to the age of 26 to remain on their parent’s medical care plan, Medicaid expansion, and initiation of the employer mandate and the individual mandate (Garrett & Gangopadhyaya, 2016). The ACA also includes new approaches to mitigate costs and enhance quality, centered on Medicaid and Medicare populations. These entailed lowering payments to healthcare facilities for certain Medicare services as well as fostering experimentation with novel delivery and payment models (Eibner, n.d.).
The ACA has affected nursing practice, emphasizing outcomes, thus placing more pressure on nursing professionals to provide adequate care. There is a need for nurses with higher education levels, for example, nurse practitioners (NPs) and advanced practice nurses (APNs). Higher education elves are necessary to ensure nurses practice better within the workplace (Nurse Journal, 2021). Regarding nursing responsibility and role, nurses currently prioritize prevention and wellness over an illness. This implies that preventive- and wellness-centered evaluations, health and wellness education initiatives, and programs addressing social and environmental drivers for preventable diseases and care issues are taken on an equal value of focus as the illness-centered clinical intervention that hospitals deliver. The nursing role within the transformed healthcare sector is anchored on the patient (Salmond & Echevarria, 2017). This means that nurses should establish a collaboration amongst practitioners, clients, and their families to ensure care choices uphold patient’s autonomy and that patients have the support and education they require to make effective decisions and influence their care.
How Quality Measures and Pay for Performance Affect Patient Outcomes
The impact of pay-for-performance programs on patient outcomes is uncertain. This implies that pay-for-performance programs have failed to consistently show positive associations with enhancing health outcomes within any given setting. Within the hospital setting, low-strength literature showed that pay-for-performance system...
