Purpose
The purpose of the graded collaborative discussions is to engage faculty and students in an interactive dialogue to assist the student in organizing, integrating, applying, and critically appraising knowledge regarding the diagnosis and treatment of mental health disorders. Scholarly information obtained from credible sources and professional communication is required. Application of information to professional experiences promotes the analysis and use of principles, knowledge, and information learned and related to real-life professional situations. Meaningful dialogue among faculty and students fosters the development of a learning community as ideas, perspectives, and knowledge are shared.

The psychiatric mental health nurse practitioner (PMHNP) is responsible for creating and maintaining a therapeutic environment with established boundaries and a trusting therapeutic alliance. The boundaries and rules of the therapeutic relationship are known as the frame. Common elements of the frame include the process for contacting the provider, what to do in case of an emergency, adherence to a schedule, fees, confidentiality, and boundaries of the therapeutic relationship. Other elements of the frame may include minor details such as whether eating and drinking during the session are appropriate, whether session interruptions are allowed, whether phone calls between sessions are acceptable, and starting and stopping on time.
Imagine you are establishing a frame for a therapeutic relationship with clients. Follow the directions below to complete your initial discussion post.
Application of Knowledge: The student post demonstrates the application of knowledge.
Describe at least TWO elements your frame would include. Provide support from at least one scholarly source.
Describe how your frame might be altered for ONE of the following situations:
your client is receiving court-mandated therapy
your client is a minor
you are providing therapy via telehealth
your client has a history of making threats against their former therapist
Identify at least TWO strategies that meet the ethical and legal requirements for a client who frequently misses appointments without notice and has failed to pay for the last four sessions. The client has received psychotherapy for six months.
Integration of Evidence: The student post provides support from a minimum of one scholarly in-text citation in addition to the textbook.
Engagement in Meaningful Dialogue: The student responds to a student peer and course faculty to further dialogue.
Substantive posts contribute new, novel perspectives to the discussion using original dialogue (not quotes from sources).
Student responds to at least one peer.
Student responds to all direct questions posed by faculty.
Post includes evidence from at least one scholarly resource to support interactive dialogue.
Professionalism in Communication: The student presents information in a logical, meaningful, and understandable sequence, and is relevant to the discussion topic.
Grammar, spelling, and/or punctuation are accurate.
Good writing calls for the limited use of direct quotes. Direct quotes in discussions are to be limited to one short quotation (not to exceed 10 words). The quote must add substantively to the discussion.

A psychiatric mental health practitioner should create and sustain a therapeutic environment during therapy. Such an environment must involve established boundaries and an alliance that espouses trust. This is known as the frame. An effective frame entails several components, which include adherence to a schedule and confidentiality.
Adherence to a Schedule
There has been a dramatic escalation of mental health disorders, in the world, in recent years. The disorders tag along several challenges, including impairing the functionality of individuals and being costly. The other prominent challenge common during therapy is appointment adherence, which makes it difficult to manage chronic and episodic disorders (Gandy et al., 2019). Those susceptible to missing appointments and failing to adhere to the outlined schedule of care are individuals with serious mental issues. This complicates matters since missing therapy appointments leads to a significant deterioration of their mental conditions. Since adherence to schedule results in optimal clinical outcomes, professionals must improvise mechanisms of improving attendance. One such mechanism is patient-centered care, which recognizes that having a holistic understanding of patients can lead to better outcomes (Gandyet al., 2019). Regarding improving attendance, clinicians can use available resources such as transport vouchers, orientations, and home visits.
The other crucial element is confidentiality, which is enshrined in the concept of informed consent and the principle of autonomy. The principle outlines the importance of respecting individuals, more so mental health patients (Darby & Weinstock, 2018). In this regard, it is critical to maintain the confidentiality of patients and safeguard their privacy. This is because they encounter such challenges as great suffering and stigma. Since most demonstrate a high degree of adaptation and strength, it would only be fair for clinicians to reciprocate by maintaining confidentiality. However, there are extenuating circumstances where psychiatrists might breach confidentiality. They include when a patient poses a danger to others or themselves. In such situations, clinicians should inform patients appropriately.
