Over the last decade, the United States has witnessed a significant focus on transitioning the health care system from volume to value-based (Spruce, 2015). For instance, there has been the increased application of evidence-based care practice with the sole objective of benefiting health care stakeholders, including various health systems, providers, and payers. Notably, these overarching changes are aimed at lowering the cost of health care and improve the health outcomes of patients. To achieve a value-based health care system in the United States, analysts have proposed a bottom-up approach that requires the participation of care systems, providers, and payers in overcoming the challenges and biases (Mamidi et al., 2020). However, incentive misalignments and competing visions and priorities of health care administration have achieved the goals a challenge (Spruce, 2015). This paper compares and contrasts the competing priorities of health care administration among different players, explores some of the areas where stakeholders are likely to conflict, and identifies how the conflicts could present in the delivery system.
Competing priorities and visions of health care administration among key stakeholders is not a novel concept in the United States. There is always an ongoing dilemma among the health care management teams in balancing conflicting or competing visions. For instance, health care providers encounter difficult decisions of providing high-quality and safe care while maintaining a certain budget and practicing within the law. Health care administrators have to observe different indicators within the system including quality measures, patient needs, financial performance, national targets, and the drive to innovate (Salmond & Echevarria, 2017). Besides, the changing health care needs, increasing expectations among the public, and ambitious health goals set by governments and international organizations have raised the bar for health care systems to provide interventions that will be of greater social value in addition to quality and safety (Kruk et al., 2018). While the goals to improve care are shared among all stakeholders, health care systems and providers will need to ensure that the interventions are affordable within their capacities.
The proposal by the Republican administration to appeal the Affordable Care Act of 2010 is one area of conflict among stakeholders in the US health care system. While the ACA has transformed access to care and overall coverage among the uninsured, the Republicans’ proposal to repeal the health care legislation would have sweeping changes to the public health care system that had ensured cover to 16 percent of Americans who were hitherto uninsured (Glied & Jackson, 2017).
