The nursing profession entails providing a holistic environment for patients to open up and express their health issues. Therefore, this essay presents a case of Jessica, a 32-year-old patient who suffers from blurred vision and pain in the eyes. Consequently, there is a detailed account of her comprehensive history and physical examination, and a highlight of the physical findings which will help in presumptive nursing diagnosis.
History Report
Jessica developed blurred vision in the morning, and the condition worsened as the hours progressed. However, she did not encounter any form of trauma or injury. Besides, there was no exposure to chemicals or a history of red eyes. She does not have a history of eye disorder but had once experienced a blurred vision a month ago, which she attributed to extreme temperatures since the vision returned to normalcy when she went to a cool environment with air conditioner. Jessica wanted an emergency check-up because her left eye’s vision level had decreased. Diabetes gets ruled out since she denies experiencing common symptoms such as fatigue, polyuria-excessive urination, dysuria, polydipsia-excessive sweat, night sweats, polyphagia-excess hunger, and weight loss (Rashid & Devaraj, 2018). She denies common symptoms of hypertension, including headaches, shortness of breath or cough, chest pain or palpitations. Other common conditions that she refuses are sore throat, constipation, swelling in the legs, congestion in the nasal or sinus cavity, vaginal discharge, pain or stiffness in the neck region, chills, fever, and abdominal pain. Equally, there is no reddening of the eye or exposure to any form of a chemical.
Physical Examination Exhibit
Jessica had a normal body temperature of 98.5F, white sclera, visual acuity in the left eye was 20/200, while at the right eye, and it was 20/30. However, her visual fields were intact on the right eye, and they could not get assessed on the left eye. The muscles had regular bulk and tone, the conjunctiva was clear, and the pupil’s ability to respond to light became reduced in the left eye, whereas in the right eye, it was brisk.
