Every 10 years, The U.S. Department of Health and Human Services and the Office of Disease Prevention and Health Promotion release information on health indicators, public health issues, and current trends. At the end of 2020, Healthy People 2030 was released to provide information for the next 10 years. Healthy People 2030 provides the most updated content when it comes to prioritizing public health issues; however, there are historical contents that offer a better understanding of some topics. Disaster preparedness is addressed in Healthy People 2030, but a more robust understanding of MAP-IT, triage, and recovery efforts is found in Healthy People 2020. For this reason, you will find references to both Healthy People 2020 and Healthy People 2030 in this course.

The recent calamity in Valley City that involved an oil tanker train derailment revealed the community's susceptibility to disaster and its adverse effects. The disaster also exposed the numerous weaknesses in the current disaster recovery plan at the Valley City Regional Hospital. The use of insufficient protocols at the hospital needs to be clarified. Lack of adequate recourses led to the overwhelming of staff. The problem was compounded by poor preparedness of the public, lack of adequate resources, and poor communication between the local authorities and the relevant stakeholders further compounded the problem. Therefore, it is essential to get an improved disaster recovery plan.
The improved disaster recovery plan is developed in the matrix of the MAP-IT framework. This framework, if implemented successfully, will assist in fulfilling the objectives of Healthy People 2030 and Healthy People 2020. The first component of the MAP-IT framework is mobilization. It will entail mobilizing the various stakeholders and creating a synergic working environment. There are several acts to be complied with in the disaster recovery plan. These acts are the Disaster Recovery Reform Act (DRRA), the Stafford Disaster Relief and Emergency Assistance Act, and the Americans with Disabilities Act (ADA).
The second component in the MAP-IT framework is assessing. It involves highlighting the areas with the greatest needs in the community, the available resources, and any other asset that can assist in addressing such areas. Based on the latest census report, the population of Valley City is increasing exponentially. Approximately 17.1% of the population is below 18 years, and 22% is above 65 years. Therefore, in case of a disaster, over a third of the population of Valley City will require special attention in terms of treatment and evacuation.
The residents of Valley City are 93% white, 3% Latino, 2% African-American, 1% Native American and 1% other races. There is also an unknown number of undocumented migrant workers with limited proficiency in English. Therefore, a practical translation technique/ tool is necessary for a disaster. Amongst the residents, 204 are elderly with complex health conditions; 147 are physically disabled or use lip-reading or American Sign Language to communicate. Therefore, special equipment (electric wheelchairs) must be present during the evacuation process for the physically disabled. For the elderly with complex health conditions, specialized health personnel must be present.
In Valley City, Resources are scarce. The Happy Valley City Shelter for the homeless runs at total capacity runs at full capacity and is not capable of accommodating the ever-increasing number of homeless people. In addition, the city is in the middle of a financial crisis, and bankruptcy is setting in. Consequently, layoffs have happened in the fire and police departments. Therefore, in a disaster, homeless people would not find any shelter in the aftermath. Laying off employees in the police and fire departments means that the staff would be severely stretched in case of a disaster.
