Background -A Middle-Aged Caucasian Man with Anxiety
53-year old Maria Perez reports that she has had alcohol problems since her late teens, while the opening of a “Rising Sun” casino near home is now her favorite location as she gambles in the casino. Perez has also gone into debt because of her gambling addiction of $50,000. Diagnosis for the patient is co-morbid addiction (ETOH and gambling.
Decision Point One-
In decision point one the choice made is “Naltrexone (Vivitrol) injection, 380 mg intramuscularly in the gluteal region every 4 weeks”. Naltrexone injections FDA approved for alcohol dependence and prevention of relapse to opioid dependence (Jones et al., 2015). I chose this option as it is one of the effective treatments for alcohol-dependent patients and it is a long-acting medication. The opioid receptor antagonist is safe and effective for alcohol treatment and opioid dependence to reduce severity characteristics (Smith-Bernardin et al., 2018).
I hoped to reduce alcohol consumption ad deal with the urge to gamble, which world reduce her addiction problems to improve her quality of life. The two options not chosen are “Antabuse (Disulfiram) 250 mg orally daily” and “Campral (Acamprosate) 666 mg orallythree times/day”. There were unexpected results as she is still smoking and has anxiety problems, as I expected that there would be improvement, and this would not concern her.
Age and behavior affect the client’s pharmacokinetic and pharmacodynamic as the client is 53 years old and she has battled gambling and alcohol addiction. There are age related changes where there is a slowdown in absorption and metabolism of the drugs. On the other hand, alcohol also alters absorption and clearance as well as increase intoxication. Nonetheless, impulsive behavior in addiction does not have a direct impact on the treatment
