Case: Robbie
• A 14-year old boy is hospitalized for relapse of leukemia after a failed bone marrow transplant. The family has been told his condition is terminal, there is nothing more to be done except keep him comfortable.
• His mother is distant, uncommunicative and rarely visits. When she does, she doesn't touch Robbie or speak to him. She sits in a recliner by the window reading novels and requesting cokes multiple times per visit.
• The staff calls a family meeting but the mother is conspicuously absent.
• Robbie's father is frantic, demanding further treatments, regardless of low likelihood of efficacy. During gthe family meeting, he directs the team NOT to tell Robbie he might die, as he would lose hope. He states:
"In our culture, we keep this news from patients to avoid upsetting them."

The case of Robbie suffering from Leukemia is critical since the family has not decided on a joint decision. When Robbie asks me if he is dying, I will tell him no, there is a treatment that will help him. He is 14 years and a nurse, and I don't have to make him lose hope. I will help him while still giving him hope regardless of the situation. This situation has several options for a nurse, and I would consider continuing to treat Robbie irrespective of the outcome. It is disadvantageous to the family since it might be costly. However, the advantage is Robbie will have to live longer, and maybe he can get better. The second option is to refer Robbie to another advanced health care, and the advantage is that it can change the mother's attitude, and probably Robbie can heal. However, the disadvantage is it might be expensive for the family.
