CASE STUDY
MODULE 2 case studyDr. Yarnell practices in pain and intervention medicine and he has worked with and supervised PA Brian for sevenyears prior to this case being brought before the board. Dr. Yarnellwas in a car accidentin 2000that left him with non-radicular cervical and back pain. He had a cervical fusion for fracture stabilization and reduced mobility. Out of convenienceandtrust Dr. Yarnell askshis PA Brian if he would treat him. The PA having a good relationship with Dr.Yarnell didn’t see any issue with this and started a medical chart. Dr. Yarnell asked to be prescribed Percocet for intermittent pain and mentioned he also had a historyof insomnia,so could Brian prescribe something for sleep. So Ambien was prescribed as well. So the prescriptions werewritten and Dr. Yarnell was diagnosed with DDD, cervical and lumbar pain and insomnia. Physical exam was noted with reduced mobility. No urine drug screen or notation of dose or interval frequency of medication prescribed was noted. Dr. Yarnell was seen every month with the exact same copy of the chart noted. A year went by and Dr. Yarnell mentioned to the PA hehad a history of fatigue and Ritalin helped him with that in the past. Dr. Yarnellrequested a prescription andthe PA added fatigue to his diagnosis. Another year went by and it was noted in the chart that Dr. Yarnell was concerned about low testosterone andhadsymptoms of low testosterone and requested a script for Androgel. This was noted in the chart with no labs being done. Over the four years since the initial visit Dr. Yarnell was consistently taking Percocet, Ambien, Ritalin, and Androgel. It was also noted that Dr. Yarnell doubled the quantities of Percocet and Ritalin and had multiple early refills on the medication which the PA prescribed. Later in front of the board, Dr. Yarnell admitted that he approached his PA out of convenienceonly. He told the board it was his idea to have his PA treat him and write the prescription and apart from thiscase his documentation and standard of care is exemplary. Dr. Brian states that he was never pressuredor concerned and Dr. Yarnell is a good physician and he was aware of his pain due to working with him on a daily basis and agreed to treat him out of convenience.

Background to the Case
Dr. Brian has been working under Dr. Yarnell who has specialized in pain and intervention. The two have been working together for more than seven years. The supervising physician was involved in an accident that led to complications.
