1. Introduction to the patient
a. Patient demographics
The Patient is 89-year-old male who is African American in descent. He was born in California and grew up in the same area. He is also married with three children and seven grand children.
b. Admitting and other diagnosis
The emergency department (ED) admitted the patient with chest pain and automatic implantable cardiac defibrillator (AICD) firing it was a mechanical complication of the device. The Active problem from the patient’s history is that he is at risk of congestive heart failure (CHF), Coronary artery disease CAD and arthritis. This means that the patient is exposed to chronic illnesses.
At the time of admission by the ED while conscious and the symptoms of the patient included vomiting, nausea, and shortness of breath. The patient was also admitted with a c/o AICD firing. The AICD was implanted in 2007; the patient states that this is the first time it has fired since he got it. He was detected to have Ventricular Tachycardia/Torsade. The day he was transferred to the coronary care unit CCU floor the patient started complaining of pain in the right upper quadrant RUQ. It is worth noting that RUQ pain may be caused by a variety of factors such as age and sex. On close examination the Ultra Sound and CT scan discovered acute Choleycystitis with no biliary association. Patient has an active problem of Coronary artery disease (CAD), unspecified congestive heart failure CHF and Ischemic Cardiomyopathy. These can be associated to aging as they are chronic illnesses that develop as the years pass on (Tracy, 2003).
c. Hospital course
The hospital course was to diagnose and treat any ailments discovered and manage the patient’s health. The age of the patient is a significant factor when diagnosing and understanding the patient’s problems. Indeed, some complications develop with age.
d. Developmental stage
The patient is at the Integrity Vs despair development stage which is the eighth stage of Erik Erikson’s psychological stages of development. The major issue at this stage is the reflection on whether they lived a meaningful and fruitful life. All through this time, the persons reflect back on how they have lived and establish a sense of pride due to the achievements and fulfillment since they have greatly achieved or regret and despondency over a misspent life. His achievements are his only factual issues and remain a great encouragement if any. Though this stage may not be liked by many its basic virtue is wisdom.
e. Family Resources
Patient resides with his wife of 42 years. He has three children and seven grandchildren. Two of his daughters live in Roanoke as well.
