For this assignment, you will develop a practice model of care to address a gap in community oral health that you have identified. Your plan should include what resources will be needed to make your plan a reality and any issues that may prevent your plan from working.
Question #1: Where would you like to work and seek employment opportunities (e.g., Long-term care facility, mobile dental practice, FQHC, SNF, school district, Head Start, etc.)? LONG TERM CARE FACILITY
Question #2: What patient population(s) do you plan to serve (e.g., pediatric, special needs, geriatric, low-income (i.e., Medicaid), homebound individuals, etc.)? GERIATRIC
Be sure to:
- Address the social determinants of health, safety net needs or resources, as well as, the current state of access to care - what services are needed and what will your plan provide?
- Discuss the possible barriers, financial and otherwise, that your target population may have in trying to use or benefit from your proposed plan.
- Identify barriers you may face in delivery of oral health care education and services through your model.

I want to work in a long-term care facility because it offers a chance to stay close with patients and assess their recovery process. It also helps professionals to make critical milestone careers as they get hands-on experience within the facility. Several health challenges within the geriatric population are reported to increase mortality rates. While many facilities are offering geriatric services, they are hardly prioritized due to the low patient-caregivers ratio. Moreover, the risks with the population make many professional caregivers and nurses avoid working in geriatric facilities.
Working for this population will allow me to understand their primary and identify the best approaches to address them. Aged people do not have the resources and physical energy to visit healthcare facilities. The vulnerability is enhanced by the lack of basic health insurance like Medicare and Medicaid. The major barrier to addressing the needs of aged people is getting sufficient financial support. Moreover, some fewer people volunteer in geriatric facilities because there are no sufficient resources to facilitate them.
The primary health challenges experienced by the geriatric population are oral health problems.
