One example of interprofessional collaboration I have witnessed in the diabetes practice is multidisciplinary rounds. The interprofessional collaboration involves different professionals from different specialties coming together to discuss, in real time, a patient’s plan of care (Lin et al., 2020). For example, I have witnessed a team of professionals including physician, fitness professional, dietician, and social work collaborate in ensuring the overall patient care. The position and context of the collaboration involves ensuring that a clear understanding of the patient needs and developing effective goals (Müller et al., 2018). The multidisciplinary rounds help develop patient-centered care that recognizes the overall needs of the patient. It is through the multidisciplinary approach that the specific health needs and desired outcomes become clearer.
Another example of interprofessional collaboration in my practice occurred with a patient from a Hispanic community. The physician was treating a 70-year-old patient from the Latino community. The physician recognized the need to provide additional support depending based on the patient demands. The 70-year-old patient informed the physician that she would be seeking assistance from a traditional healer famed in her community for healing diabetes. The physician agreed and even collaborated with the traditional healer a few months after treating the patient. In this situation, the goal of the physician was to give the patient peace of mind and ensure that the treatment modality is followed (Moss et al., 2016). The physician aimed at ensuring that the patient felt comfortable using herbal treatment in combination with modern medicine.
One of the things that was alike in the two scenarios is evidence of team-based care. A team-based perspective allows interprofessional collaboration by respecting and working together to improve patient outcomes (Moss et al., 2016). The collaboration helps reduce inefficiencies by conducting assessments and treatment together to focus on the patient as a whole rather than working on a specific diagnosis (Müller et al., 2018).
