Subjective information and documentation
The subjective data depends on the affected area and can only be described by the person affected and this was explained by the complaint that she had bumps on her bottom. Subjective data is imperative and besides the patient’s symptoms, the patient talks about her feelings, perceptions and concerns. The patient’s medical history is considered to determine whether here are aggravating factors and how long the symptoms have progressed. A pap smear exam was undertaken s 3 years ago, and there was no dysplasia, but she was treated for Chlamydia after diagnosis about 2 years ago.
AB reported the presence of the bumps, but needs to provide more details on whether there have been changes in the size, color, pattern and size, since she first noticed the bumps. Additionally, the subjective data on whether AB has noticed the bumps anywhere else besides her bottom on her both and if they are itchy is helpful to make a proper diagnosis and assessment. If the bumps are spreading and depending whether they appear all the time or diaper this helps to compare health outcomes with other regions of the body. It is likely that there are chemicals, soaps and garments that aggravate the patient’s condition, and if she is aware of any such factors.
Objective information and documentation
The genital/recta and urinary objective data are important when evaluating the patient for the possible cause of the bumps. While AB had a pap smear 3 years ago, there have been no further paps and since she has a history of Chlamydia this is considered in the objective data. More data would focus on other areas of the genital/rectal area including the cervix, ovaries and any unusual changes in the uterine area. The pap smear tests whether the cervical cancer cells are present and there is no information on whether there is history of abnormal pap smears among the patient’s family members.
An examination of the external and the internal genital is often conducted to make a more accurate diagnosis. For instance, the pap smear the wet prep/ mount test and culture may be used when testing for yeast, bacteria, and trichomonas and determine whether the vaginal specimen is normal. Additionally, the rectal examination assesses sphincter tone and whether there are any masses, polyps, or nodules, lesions, strictures, tenderness, trauma and hemorrhoids.
