Monitoring and Management of Amyotrophic Lateral Sclerosis (ALS)

Research Paper Instructions:

Write a paper of 500-750 words (not including the title page and reference page) on your proposed problem description for your EBP project. The paper should address the following:
1.Describe the background of the problem. Tell the story of the issue and why it deserves attention. Identify the stakeholders/change agents and who or what organizations are concerned, may benefit from, or are affected by this proposal. List the interested parties, patients, students, agencies, Joint Commission, etc.
Use the feedback from this week’s (Week 2) Discussion Board post, and refine your PICOT question for the final proposal. Make sure that the question fits with your graduate degree specialization.
(My Degree specialization is Adult Geriatric Nurse Practitioner (AGNP)
This is my PICOT question: In patients diagnosed with Amyotrophic Lateral Sclerosis (ALS), (Problem) how does the use of telemedicine monitor and manage (Intervention) compare to in person clinic visits (Comparison) postpone disease related complications (Outcome) over 3 months after intervention.
2.State the purpose or goal and project objectives in specific, realistic, and measurable terms. The objective should address what is to be gained. This is a restatement of the question, providing focus on how to answer the question.
3.Provide supportive rationale that the problem or issue is an important one for nursing to resolve; use relevant professional literature sources.
4.Develop an initial reference list to assure that there is adequate literature to support your evidence-based practice project. The majority of references should be research articles. However, national sources such as Centers for Disease Control and Prevention (CDC), National Center for Health Statistics (NCHS), Department of Health and Human Resources (HHS), or the Agency for Healthcare Research and Quality (AHRQ), and others may be used when you are gathering statistics to provide the rationale for the problem.
Once you get into the literature, you may find there is very little research to support your topic and you will have to start all over again. Remember, in order for this to be an evidence-based project, you must have enough evidence to introduce this as a desired practice change. If you find that you do not have enough supporting evidence to change a practice, further research would need to be conducted. The grading guidelines for this assignment are in the rubric section of this syllabus. Upon receiving feedback from the instructor, refine Section B: Problem Description for your final submission. This is a continuous process throughout the course for each section.

Problem Description
Background of the Problem
The pandemic has presented many issues concerning the accessibility of health care. This is especially true for geriatric patients and those with conditions that require regular monitoring, such as Amyotrophic Lateral Sclerosis (ALS). Luckily, the advancements in technology led to telemedicine's availability, but there are still concerns regarding the effectiveness of this approach compared to face-to-face hospital/clinic visits and medical consultations. The American Telemedicine Association raises its concern about possible issues with monitoring the quality and standard of care provided to the patients because it is different from the usual provision of health care needs (Nesbitt et al., 2012). The stakeholders involved in the study are the medical professionals and patients with ALS who are the ones that will be directly affected by the effects of the intervention.
PICOT Question
In patients diagnosed with Amyotrophic Lateral Sclerosis (ALS), (problem) how does the use of telemedicine monitor and manage (intervention) compare to in-person clinic visits (comparison) postpone disease-related complications (outcome) over three months after intervention?
Purpose and Project Objectives
This study aims to compare telemedicine's effects with face-to-face medical consultations for the regular monitoring and management of patients with Amyotrophic Lateral Sclerosis (ALS) by the prevention of ALS-related complications in 3 months. The intervention includes digital speech analysis, remote ventilatory monitoring, self-reported ALS functional rating scale, neurological assessment through mobile applications, and videoconferencing telemedicine consultation (Bombaci et al., 2020). In addition to these, remote cognitive assessment, psychological counseling, and a nutritional status check will also be provided as part of the intervention for a holistic evaluation and treatment for ALS patients (De Lucia et al., 2020). Some of the possible ALS-related complications that are looked out for are aspiration, loss of function, weight loss, pressure ulcers, and lung failure.
Rationale
This research study will support the innovation of the current health care system involving adult geriatric nurse practitioners and other health care workers involved in the assessment and treatment of ALS patients. It will be beneficial in decreasing health risks for the health care providers, especially during the pandemic, while also increasing the accessibility of vital assessment and treatment for ALS conditions.
