An activity I have observed that reflects interprofessional collaboration (IPC) is in the development of a patient’s individualized plan of care. A team of professionals including a physician, nurse, physical therapist, dietician, fitness professionals, and pharmacist worked together to develop individualized statement plans for a patient with diabetes. In this case, the patient was an amputee because of diabetes complications and the team worked together to develop a plan of care.
The dynamics of the observed IPC is patient-centered care. QSEN (2020) notes that patient-centered care recognizes the patient is the source of control and professionals should partner to offer compassionate and coordinated care by respecting patient needs, values, and preferences. Patient-centered considers the cultural, ethnic and social background of the patient during the development of the care plan (QSEN, 2020). The team of healthcare professionals I observed understand that patients have unique needs that would best be understood through a collaborative approach. In this regard, patient-centered care seeks to work with patients and empower them to take part in their health goals.
One way I am developing interprofessional collaboration skills in my practice is through roles and responsibilities. I focus on learning how to describe my professional role and responsibilities to team members as a way of preparing myself in collaborative practice. Whenever I have meetings with other professionals working with people with diabetes, I take time to explain my roles so that team members have a clear understanding of my expertise and capabilities (Interprofessional, 2011). I know I cannot collaborate in patient care if the team does not know my capabilities or if I do not understand those of the team.
