INTRODUCTION
Social determinants of health (SDOH) are complex and multilayered, and they are affected by a variety of variables. SDOH are often difficult to address and require equally complex, multilayered partnerships designed to correct their trajectory by addressing the variables that underpin them. Clinical care alone cannot ensure the optimal health of the client without addressing SDOH.
Engaging with community partners is a promising method for influencing the effect of SDOH on the health of the community’s citizens. Successfully sustaining that engagement requires strong communication skills and developing strategies designed to motivate the community partners to continue collaborating with those in a health or behavioral health practice setting.
For this task, you will create an outreach presentation for your organization that will be given to members of the community.
SCENARIO
You are employed as a health services coordinator (HSC) in a health or behavioral health practice setting in your community. You are developing a presentation for community partners on the SDOH. This presentation describes your outreach methodology to gain the engagement of a selected typical and nontypical community partner in this work with your organization.
REQUIREMENTS
Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. The originality report that is provided when you submit your task can be used as a guide.
You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.
Using the information in the scenario, design a multimedia presentation (e.g., PowerPoint or Keynote) (suggested length of 15 slides and 15–20 minutes) to present to your organization, with speaker notes, that describes your outreach plan for the community by doing the following:
Note: The audiovisual recording should feature you visibly presenting the material (i.e., not in voiceover or embedded video) and should simultaneously capture both you and your multimedia presentation.

3. Describe a community partner who might not typically be engaged in addressing the targeted SDOH and with whom you wish to engage in addressing the SDOH from part B2 with your organization.

Social determinants of health (SDOH) refer to a set of conditions in which people are born, grow, educated, work, and age. Examples of SDOH include socioeconomic status, neighborhood & physical environment, social support network, and employment. In modern public health systems, the utilization of data has enhanced the ways in which SDOHs are identified and addressed; as part of evidence-based best practice. Housing and labor data, for instance, reveals whether people have housing or not (homelessness), quality of employment choices (household income), and quality of education (Hinton & Artiga , 2018). The three, homelessness, household income, and education are key social determinants of health. Since people living in low-income areas or undergoing homelessness experience more obstacles in access to health (Lazar & Davenport, 2018), housing data from the United States Census Bureau (USCB) can be used to compare the safety of housing or neighborhood with existing and perceived health risks. Similarly, research indicates that household income (Lenhart, 2018) and family’s level of education (Zajacova & Lawrence, 2018) also influence individual and community health outcomes. Patient Zip data can be coupled with labor or housing data to establish whether certain conditions or ailments are consistent with income or level of education. Homelessness is the condition of lacking safe, adequate, and stable housing. Homeless people suffer from higher rates of illnesses, have little or no access to care, have little or no social support, and die from conditions that are otherwise preventable and treatable. However, since the issue of homelessness affects a vast number of stakeholders, solutions are increasingly undermined through politicization, discrimination, and lack of empathy. One strategy for mitigating homelessness is the Housing First Approach (HFA). According to Tsemberis (Tsemberis, 2017), most, if not all, contemporary approaches to solving homelessness focused on treatment first before housing. In other words, the HFA focuses on direct access to housing, providing a stable base from which homeless people can rebuild themselves. (Aubry et al., 2019) conducted a controlled trial of Housing First's effectiveness in a small city in Canada. They established that participants entered the program more quickly (within 23 days as opposed to 88 days), remained stably housed for a longer period, and showed greater gains in their quality of life.
