INTRODUCTION
Social determinants of health (SDOH) are complex and multilayered, and they are affected by a variety of variables. SDOH are often difficult to address and require equally complex, multilayered partnerships designed to correct their trajectory by addressing the variables that underpin them. Clinical care alone cannot ensure the optimal health of the client without addressing SDOH.
Engaging with community partners is a promising method for influencing the effect of SDOH on the health of the community’s citizens. Successfully sustaining that engagement requires strong communication skills and developing strategies designed to motivate the community partners to continue collaborating with those in a health or behavioral health practice setting.
For this task, you will create an outreach presentation for your organization that will be given to members of the community.
SCENARIO
You are employed as a health services coordinator (HSC) in a health or behavioral health practice setting in your community. You are developing a presentation for community partners on the SDOH. This presentation describes your outreach methodology to gain the engagement of a selected typical and nontypical community partner in this work with your organization.
REQUIREMENTS
Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. The originality report that is provided when you submit your task can be used as a guide.
You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.
Using the information in the scenario, design a multimedia presentation (e.g., PowerPoint or Keynote) (suggested length of 15 slides and 15–20 minutes) to present to your organization, with speaker notes, that describes your outreach plan for the community by doing the following:
Note: The audiovisual recording should feature you visibly presenting the material (i.e., not in voiceover or embedded video) and should simultaneously capture both you and your multimedia presentation.

To access Panopto's website, navigate to the web link titled "Panopto Access", and then choose to log in using the “WGU” option. If prompted, log in using your WGU student portal credentials, and then it will forward you to Panopto’s website.
To submit your recording, upload it to the Panopto drop box titled “Community Presentation” – GFM1 | D057.” Once the recording has been uploaded and processed in Panopto's system, retrieve the URL of the recording from Panopto and copy and paste it into the Links option. Upload the remaining task requirements using the Attachments option.
A. Create an audiovisual recording of yourself delivering your presentation, using the Panopto system and incorporating your multimedia presentation.
B. Analyze significant SDOH in your community by doing the following:
1. Describe how you used data to identify three SDOHs that are affecting your community, including the sources of the data.
2. Describe a strategy for mitigating one SDOH from part B1, using one credible data source to support your choice of strategy.
3. Explain how targeting the SDOH from part B2 aligns with your organization’s mission.
4. Discuss the effects of sociopolitical drivers and cultural diversity on the SDOH from part B2 at the mezzo and macro level.
C. Discuss the targeted community partners that you wish to engage to assist your organization in addressing the chosen SDOHs by doing the following:
1. Describe two engagement strategies you will use in the presentation to capture the interest of community partners.
2. Explain why a specific community partner would be beneficial to engage in this work with your organization.
3. Describe a community partner who might not typically be engaged in addressing the targeted SDOH and with whom you wish to engage in addressing the SDOH from part B2 with your organization.
a. Justify your selection of the nontypical community partner from part C3.
4. Discuss two strategies to sustain the engagement of the partners from parts C2 and C3 in addressing the SDOH from part B2.
5. Discuss the effect of successfully engaging the community partners from parts C2 and C3.
D. Discuss opportunities for community alignment to enhance the work within your organization by doing the following:
1. Discuss two ways of working with the community partners that could contribute to the professional development of organizational staff members.
2. Describe two common organizational processes which may enhance the work of the organization with the community partners.
3. Discuss how you will use two methods to measure the success of your community outreach program.
E. Acknowledge sources, using APA-formatted in-text citations and references, for content that is quoted, paraphrased, or summarized.
