To the C.E.O,
Introduction
Due to the unfortunate event at CNRL farm, a company with similar operations as ours, I want to review the incident report and give recommendations as a health and safety officer. The aim of this is to provide a precautionary approach to any eventuality. The paper also investigates the root cause of the incident and any relevant information.
Review of the incident
Workers crushed by the tank roof support structure (8) describe the event as an unfortunate one that occurred on the 24th of April 2007 at CNRL Company. The collapse was initiated by wind action on the roofing structure of the oil tank that was still under construction. The report's case fatality was that two individuals received major injuries while three others received minor injuries. One was declared dead at the construction site of the two that passed on.
The nature of the fatalities and the injuries indicate that the accident was a severe one. It, therefore, prompts one to ask themselves questions of necessity. Some being, were due processes put in place at the construction time? Did personal protective equipment properly protect the workers? Have all the due legislation adhered to? From these, one can conclude and provide the necessary recommendation.
Investigators from different government agencies were sent to look into the matter. They first did the necessary thing of issuing the order to stop employees of CNRL from working on the tank. It ensured that investigations were done in all tank dimensions, and careful analysis revealed the causes of fatalities and injuries. A compliance officer, WHSC, sent by the Albert government, found out the following: that there were a total of thirteen workers at the time the incident occurred, and all of them were Chinese who were on temporary employment. Also found out was that the onsite fatality was the east side of the tank and was struck on the occipital bone by steel. The investigator also looked at the incidences of injuries and found out that one of the workers with major injuries was working on the welding machine and had been struck by a dislocated steeled part of the welding machine. The other worker who also had serious injuries was also working on a welding machine and had also been struck by steel that had been completely dislocated from the welding machine (Workers crushed by the collapse of tank roof support structure, 10)
The report also indicates that days after completing the investigation on the CNRL tank, a part of it was later blown away by the wind. At this particular time, no one was injured because the law enforced to restrict workers from working on the tank was still in force.
