Introduction
During a mass casualty situation, the sudden entrance of many individual people puts vast pressure on institutions to cope. Research shows that clinics have to expand the surge capacity by introducing additional resources and providing extra care to the injured people (Ahmad 2018). Sometimes, clinics do not have enough ability, materials, and resources to deal with the situation, posing serious management threats. The purpose of the paper is to address several issues, which might occur at mass causality incidents. The report will address the clinical issues, decontamination, and hazardous material issues at mass causality incidents. Based on this, the recommendations are proposed, and a conclusion is drawn.
Critical Clinical Issues
Communication is the significant internal and external risk faced by clinics during MCIs. The challenges are in the form of street violence, field triage lacking, and prehospital care absence. In such cases, the consumable supplies quickly exhaust with poor record-keeping and exhausted staff, which shows a sign of strain. Hospital safety is challenging because of rising tensions within the premises and the threat of external attacks (Hugelius et al., 2021). The patients could also suffer due to suboptimal care due to hospital supplies exhaustion.
Another possible clinical issue is that the rescue network does not perform promptly, which is related to delayed support out in the actual situation. The rescue network might lack effective communication equipment, which leads to delayed information in real situations. For example, in 2020 in Australia, over 2000 homes were destroyed in the Australian Marathon fires. It was very challenging for hospitals and governments to arrange quick emergency management due to a lack of communication and sudden planning. A million animals were dead, and serenity hospitals could not manage such a large quantity of animals (BBC 2021). Sometimes, the clinical issues are linked with a poor transferring system and uncontrollable excitement among teams.
There is a need to have an independent field commander to manage, coordinate, and give orders to teams. The field commander needs to realize roles and lead teams throughout hazardous issues. Sometimes, there is an issue of improper first aid care leaders to a poor transferring system. The teams must be authorized with clear roles and boundaries. The emergency wards should be alerted for 24 hours to manage mass incidents. The recent viral attack COVID-19 has brought several people to clinics worldwide, and clinics had to address the critical cases (Li et al., 2021). The countries and clinics with low resources suffered a lot, both financially and human loss. It showed that emergency management and resources were necessary to align clinics and services.
