The documentary was directed at the time when neurosurgeon professional had not developed the art of surgery. First, the daring and courageous physician Walter Freeman would peel open the patient's eyelid and insert an ice pick between the eyeballs and the eyelid. The doctor would tap the ice pick with a surgical hammer to break through the orbital cavity of the eye down to the prefrontal lobe. Then the doctor would shock the audience by wiggling the ice pick to split the frontal lobes and remove the ice pick thereafter. The entire documentary written and co-directed by Barak Goodman explore this shocking surgical process developed by Doctor Walter Freeman to serve the life of mentally ill patients. Freeman developed the method from Moniz of removing fixed ideas from the frontal lobes. The process was deemed quick and easily reproduced by using ice picks. Though Freeman met several criticisms for applying an untested procedure to human life, he ploughed later in the documentary by earning an approval of the press, The New York Times which eventually termed it surgery of the soul.
As the wisdom dictates, Dr. Walter Freeman gained fame for perfecting the lobotomy. He is hailed as a miraculous solution and cure for the severally mentally ill patients.
