Purpose :
The purpose of this assignment is to understand the definition of “healthy community” as identified in research and to apply theory to practice. Using the 12 Social Determinants of Health and the in class experience of the board game, The Last Straw, students will create a narrative (story) to gain a broad understanding of ‘health” as it relates to children, families and communities.

There are 12 determinants of health that are described in the article. Read the underlying premises and evidence to gain an understanding about how health is defined in Canada. Pursuing other links/reading in the article may be of interest to you to further understand key topics (e.g. how being black and female is related to health, impact of violence against women...).
After reading about the 12 determinants of health you will use these determinants to create a narrative or story about a family with young children and how their lives have been impacted by the determinants of health. You can base your story on a family you know, your own family, a family you may have met in field placement, a family based on the Last Straw character your team played in class or create a fictitious family. Use only fictitious names. You can create the story using any creative format, a picture book, a written story, a poem, a song, a rap song, a comic or e-magazine
The Story
Try to use as many as the determinants of health as you can to demonstrate your understanding of all 12 determinants. Your story should show how you are linking theory to practice to illustrate the impact of these determinants on a child (children), family and community. The story can have a positive focus, the family is living in a community wheremany of the social determinants are met or it could be negative, a family is struggling as social determinants are missing or it can be a bit of both.
The story should demonstrate the characteristics of the neighbourhood or community, its strengths, challenges, characteristics, services, employment, languages, poverty and so on. Consider demographics, ethnicity, employment, age, gender, education, the barriers or opportunities felt by the family in this neighbourhood.
The story should represent the neighbourhood as vividly as possible, and help the reader to have an understanding of what the social determinants of health look like within one family’s life.
Remember to include an introduction, body and conclusion in your writing. APA format must be used for all text and graphics.
Include a reference page and cite all sources. If you do not use any sources other than the article that is required reading for this assignment, “What Makes Canadians Healthy or Unhealthy” then only include the article in your references.

Health is a fundamental need of every human being. The conditions in which one is born works, plays, lives or learns impact the life quality, health, and functioning risks and outcomes. Some of the factors that affect the health of a population include the physical environment, education, employment conditions, social and income status, culture, and health services. The health of individuals can hence improve through ensuring that people access education, public safety, affordable housing, a safe environment, and health services. Canada should strive to ensure that the social health determinants are met to make sure that citizens live healthily. The story of Simon’s family highlighted below illustrates the extent that the social determinants can limit people from living healthy lives.
Simon is 40 years old and works as a fuel clerk. He is married to Elizabeth, who is five years younger. Together, they have two children; Dave aged 17 and Sara aged 14. Both Simon and Elizabeth are born of black immigrant parents. Simon comes from a peasant family that could not afford most of their needs and he and the siblings only got a chance to obtain only elementary education. Despite Simon being a bright kid, he could not pursue his dream of becoming a doctor. Also, Elizabeth comes from a humble household that saw her attain high school studies as her highest level of education. She was previously employed as an office messenger in a chemical company. The family lives in a poor neighborhood primarily composed of old houses that have undergone little to no maintenance for many years. There are companies within the locality that have contributed to land, water, and air pollution. Additionally, the residents have to scramble for the few resources that they have, and others have to access these from the neighboring estates that are expensive. There are two schools and one hospital that serves the whole community.
Simon’s family has been forced to contend with the challenging conditions they live in because the much they earn can only sustain their necessities. Their problems began when Elizabeth got pregnant with their first child, Dave. At the time, she was working in the chemical industry, and she got exposed to toxic substances that got her hospitalized for two weeks. Elizabeth had to quit her job after getting advice from her doctor on the dangers that chemicals posed to both her health and that of the baby. The loss of one source of income took a toll on both Simon and Elizabeth and they began drifting apart. Elizabeth had no one to lean on and resorted to drugs that could keep her mind off her problems. The low educational level meant that she could not comprehend the damage the drugs were doing to her unborn child. Additionally, the family could not afford the cost of having frequent antenatal visits. Elizabeth thus had minimal knowledge concerning the manner that a woman should carry herself during pregnancy. She did not intake the desirable nutrients because the family could not afford them and did not understand their importance.
