Why Nursing Documentation is Vital for Medicare Reimbursement

In an acute care facility, only the physician’s documentation is considered when applying for Medicare reimbursement. Why isn’t nursing documentation considered? What are some examples of where nursing documentation can be supportive of the physician's documentation? How do you see documentation as being the key to the reimbursement process? Defend your stance on the accuracy of records and how it is used as the reimbursement of different carriers including (but not limited to) Medicare.
