Page one of your submission will include your graphical display of the first layer of the logic model.
You will also include a narrative on pages 2-3 that describes the research you conducted to make these decisions and ultimately you will provide a GOAL statement for the program you plan to design. The narrative should be no longer than two pages single-spaced, and should be structured as follows:
Title: Summary of Quality of Life and Epidemiologic Priorities for Jackson County, Ohio (example) Subheading: Priority Health Issue.
In this section, pull from your community health needs assessment, and other work, to include a summary (1-2 paragraphs) of the epidemiologic evidence that substantiates your decision to select your health issue. Include sufficient data to describe both incidence and prevalence of the health issue and also identify trends related to the distribution of the health issue based upon demographics and other factors.
Subheading: Quality of Life Issues.
In this section, describe the three QOL issues that you included in the logic model and describe their relationship with the health issue. Make the case that by addressing the health issue in your program, some resolution or improvement to these QOL issues can be expected. List each QOL issue separately in a sub-heading and pull from your assessment and other work to provide a substantiation for the inclusion of each issue.
Subheading: Target Population.
Identify the specific segment of the community that will be the target of focus for your program. This should be based upon the epidemiologic evidence in your CHNA that supports the selection of this particular sub-population. If you described the epidemiologic evidence for this segment of the population in the "priority health issue" section above, you do not need to repeat it and only need to provide a clear statement of the sub-population.
Subheading: Program Goal.
Provide a single goal statement based upon the selected health issue. This will be the overall goal of the program you will develop over the course of the semester. Be sure to provide the goal in correct form and content as has been described in course materials and the course lecture.

Priority Health Issue
One of the priority health issues identified by the community health needs assessment (CHNA) document is a cardiovascular disease: the county’s mortality rate from cardiovascular disease is considerably higher than the nation’s overall mortality rate (199.8 per 100,000 vs. 100.8 per 100,000) (The CHSA Subcommittee, 2019). A 2019 health survey showed that a significant proportion of adults in the county demonstrate risk factors linked to cardiovascular disease: high blood cholesterol (39.0%), high blood pressure (37.6%), obesity (31.3%), sedentary lifestyle (25.7%), smoking (14.2%), and diabetes (12.7%) as shown in Figure 1 below.

Some of the leading types of death in Cuyahoga County are heart disease (30% of all deaths), cancers (23%), stroke (5%), chronic lower respiratory diseases (5%), and accidents together with unintentional injuries (3%). Men had higher mortality rates from heart diseases than women, while African Americans recorded the highest deaths per 100,000, as shown in Figure 2 and Figure 3 below.

The logic model above illustrates the health issue as well as three QOL issues that describe the physical health, level of independence, and psychological state experienced by patients with cardiovascular diseases. Physical health as a QOL indicator assesses the impact of cardiovascular disease on a patient's function and wellness. Individuals with the cardiovascular disease face several comorbidities like heart failure, hypertension, osteoarthritis, and ischemic heart disease, which result in poorer physical health and, in turn, a poorer level of QOL (O’Neill & Forman, 2019). Cardiovascular disease is also associated with chronic conditions including diabetes, deafness, arthritis, and asthma, all of which result in physical challenges such as dizziness; shortness of breath; weakness of limbs; abdominal pain; pain in the jaw, neck, and throat; chest pain and tightness; nausea and fatigue; as well as cold sweats (CDC, 2019).
