Topic Relevance
Electronic health records are increasingly recognized in healthcare to improve communication, safety, and patient-centeredness. Electronic health records (EHR) are used in chronic diseases such as diabetes to facilitate continuity of care and improve patient health outcomes (Lessing & Hayman, 2018). West Virginia is one of the states with the highest population of people diagnosed with diabetes (CDC, 2022). This justifies the selection of electronic health records primarily in West Virginia to help contain the rising cases of diabetes. The objective includes reducing hospitalization and the resulting costs, which ultimately invites improved healthcare outcomes in West Virginia.

Mission statement
To improve patient safety and quality care of outpatient diabetes patients.
CDC (2022) indicates that the number of patients diagnosed with diabetes in West Virginia is high. It is important to remember that diabetes is increasingly tied to other chronic conditions, including stroke and heart attacks. In some instances, the focus includes the increased rates of kidney failure and blindness or blurred vision (World Health Organization, 2022). If the cases of diabetes are not handled, it can worsen to include increased mortality rates. In fact, the US registered approximately 460 000 deaths resulting from kidney complications caused by diabetes. In West Virginia, for example, about 232 000 citizens are diagnosed with diabetes and nearly 50 000 are at risk or have diabetes that is yet to be diagnosed (American Diabetes Association, 2021). As such, there is a need to improve patient safety and quality care of outpatient diabetes patients in regions such as West Virginia. In this case, the focus is on reducing instances of hospitalization and the costs tied to care, which, in turn, yields the success of the mission. This is backed by Pulleyblank et al. (2020) who indicate diabetes among other ambulatory care-sensitive diseases are well managed by EHRs. It includes the fact that EHR can improve care provision by facilitating the exchange of patient information and improved organization. This ultimately translates to improved patient safety and quality care for outpatient diabetes patients.

The focus in the vision statement is the long-term objective. In other words, it includes the goal for the future. The local government spends a considerable amount of resources to manage diabetes. The American Diabetes Association (2021) indicates that “diagnosed diabetes costs an estimated $2.3 billion in West Virginia each year.” The organization further indicates that cases of diabetes attract other complications that increase the risk of death and other life-threatening complications such as cardiovascular diseases.
