Introduction
The SBAR template is a valuable tool in the nursing profession. According to Shahid & Thomas (2018), SBAR emphasizes clear and concise communication between medical personnel during handoffs. The key to effective and quality treatment is the provision of relevant information on time. The SBAR technique involves purposeful organization, summarization, and collaboration between teams to arrive at the ideal action. Yan, Wu, & Feng (2018) purport that applying the SBAR technique in acute pulmonary embolic cases improves the quality of medical care and fosters a culture of critical thinking for both practicing nurses and students.
During my nursing practice at Birmingham Hospital, I attended patients admitted to the hospital for treatment and observation. This essay will focus on pulmonary embolism/cardiac event cases where I utilized the SBAR technique to facilitate early treatment. This study aims to showcase the essence of good communication channels between health care officers throughout the continuity of care. The patient's name will be withheld for confidentiality purposes. I will address the patient as Jane for this essay.
Situation
The first part of the SBAR technique involves a brief presentation of what happened. During a busy night shift at Birmingham medical ward, Jane complained of sharp chest pain radiating to the left upper quadrant of the abdomen. She is experiencing sudden shortness of breath, lightheadedness, rapid heartbeat, sudden cough, spitting up blood, irregular heartbeat, respiration distress, sweating, cyanosis, Hypoxemia, and Central Cyanosis, as well as episodes of anxiety.
Background
The second part of the SBAR template focuses on the medical history of the patient. Jane has been in treatment for Covid 19 for 4days. About 4hours ago, Jane reported shortness of breath, episodes of restlessness, and complained of chest pains. Jane recalls that she felt short of breaths since taking a five-hour airplane flight 14 days earlier. In addition, Jane had earlier on been treated for only an injury in the forehead laceration. She was brought in for treatment after falling down the stairs at her home. Jane's vitals are as follows blood pressure reads 128/80 mmHg; respiration, 24 breaths per minute; pulse 120 beats per minute; temperature 98.4%, oxygen levels on room air 89%, increasing to 94% on 100% Fraction of oxygen thanks to the nonrebreathing aiding mask.
Assessment
The third part of the SBAR technique involves the assessment of the vitals, symptoms, and medical history of the patient.
