Quality Improvement Initiative: Sepsis Care Optimization to Improve Outcomes

Essay Instructions:

Assessment 2
Quality Improvement Initiative Evaluation
Overview
Overview
Deliver a 5–7-page analysis of an existing quality improvement initiative at your workplace. The QI initiative you choose to analyze should be related to specific disease, condition, or public health issue of personal or professional interest to you.
Too often, discussions about quality health care, care costs, and outcome measures take place in isolation—each group talking among themselves about results and enhancements. Because nurses are critical to the delivery of high-quality, efficient health care, it is essential that they develop the proficiency to review, evaluate performance reports, and be able to effectively communicate outcome measures related to quality initiatives. The nursing staff's perspective and the need to collaborate on quality care initiatives are fundamental to patient safety and positive institutional health care outcomes.
Context
Context
The purpose of the report is to assess whether specific quality indicators point to improved patient safety, quality of care, cost and efficiency goals, and other desired metrics. Nurses and other health professionals with specializations and/or interest in the condition, disease, or the selected issue are your target audience.
Questions to Consider
As you prepare to complete this assessment, you may want to think about other related issues to deepen your understanding or broaden your viewpoint. You are encouraged to consider the questions below and discuss them with a fellow learner, a work associate, an interested friend, or a member of your professional community. Note that these questions are for your own development and exploration and do not need to be completed or submitted as part of your assessment.
Reflect upon data use in your organization as it relates to adverse events and near-miss incidents.
• How does your organization manage and report on adverse events or near-miss incidents?
• What data from your organization's dashboards help inform adverse events and near-miss incidents?
• What additional metrics or technology are you aware of that would help ensure patient safety?
• What changes would you like to see implemented to help the interprofessional team better understand data use and data trends as quality and safety improvement tools?

Assessment Instructions
Preparation
You have been asked to prepare and deliver an analysis of an existing quality improvement initiative at your workplace. The QI initiative you choose to analyze should be related to a specific disease, condition, or public health issue of personal or professional interest to you. The purpose of the report is to assess whether specific quality indicators point to improved patient safety, quality of care, cost and efficiency goals, and other desired metrics.
Your target audience consists of nurses and other health professionals with specializations or interest in your selected condition, disease, or issue. In your report, you will define the disease, analyze how the condition is managed, identify the core performance measurements used to treat or manage the condition, and evaluate the impact of the quality indicators on the health care facility:
Note: Remember, you can submit all, or a portion of, your draft to Smarthinking for feedback, before you submit the final version of your analysis for this assessment. However, be mindful of the turnaround time for receiving feedback, if you plan on using this free service.

Sepsis Care Optimization to Improve Outcomes
Introduction
The Healthcare system faces diversified problems, for instance, infections acquired through hospital facilities, medical errors, quality care, and service delivery. Quality improvement initiative is a continuous assessment that evaluates the healthcare system's success in terms of service delivery. There are different quality improvement initiatives; however, this research study will focus on sepsis care optimization. Sepsis is a major healthcare issue that is life-threatening, and it is usually associated with acquired infections, either from hospital facilities or outside. Typically, sepsis occurs when an existing infection sparks a series of reactions in the body (Megan, 2017). It is a deadly infection; without early recognition or treatment can lead to loss of life. This study will explore different types of sepsis infection, sepsis, shock sepsis, and severe sepsis.
Furthermore, nurses play an integral role in the success of a hospital and quality improvement initiatives. There are two primary roles of a professional nurse in respect to QI, firstly, to meet the obligations of the hospital in a disciplined manner. Secondly, to control all nursing-sensitive indicators which might affect patient outcomes and ensure QI is fully implemented in the hospital (Nyström, Garvare, Westerlund & Weinehall, 2014). Therefore, the integration of quality improvement initiatives helps nurses meet the needs of patients and the hospital in general with efficacy.
Overview
Sepsis is an infectious disease, and it is grouped into three segments. Firstly, sepsis develops in the bloodstream, especially after the continuous accumulation of chemicals; as the chemical tries to fight infection, it triggers inflammation. According to the Center for Disease and Prevention Control, at least 250,000 Americans die of sepsis infections. It is a serious disease that requires a lot of attention, and it is the obligation of hospitals and stakeholders such as nurses to ensure it is controlled. Severe sepsis is an advancement of sepsis on level one and is associated with organ failure, discolored skin, mental ability, breathing problems, and heart problems (Megan, 2017). Sepsis shock is characterized by low blood pressure and severe sepsis symptoms. Mostly, sepsis is caused by various infections, for instance, kidney infections, pneumonia, and bloodstream infections. Who is vulnerable to the disease? As discussed earlier, sepsis is an infectious disease, and people with high infection rates are patients in the intensive care unit, young children and aged, people with chronic diseases due to the weak immune system, and patients with catheter tubes.
Optimization of sepsis care is a vital component in the health system because it helps in early recognition of the disease, hence improving outcomes. Sepsis is a major burden in health care due to its mortality rate; early recognition of the disease is a great challenge. It is difficult to determine patients or individuals if they have sepsis because the syndromes are non-specific. Additionally, due to poor or lack of QI, the...
