Demographic information: A.M. is a 25-year-old Caucasian female patient who lives in San Francisco, California, and was born on December 1, 1995. A.M. is a Roman Catholic. She is a Bachelor of Science in Physical Therapy graduate and currently employed as a physical therapist in a hospital and a part-time visiting physical therapist. Her income is above average.
Presenting problem: Suicidal tendencies
History of present illness: 7 months ago, A.M. started losing interest in her hobbies and passions like dancing, singing, and playing the guitar and piano. She attributed the loss of interest to their family problem because she discovered that her father was having an affair with her aunt. She explains that initially, she was planning to keep it to herself and not tell her mother and siblings. Two days after, her family found out because they saw the two themselves. She felt lonely and lacked the willingness to eat, resulting in a significant weight loss around 3 kilograms in a week. During those times, she was frequently depressed throughout the week and felt like she lost all the energy that she had before. Her friends and colleagues started worrying because she looks like she was not herself every day and got reprimanded at work multiple times due to many mistakes. Her manager talked to her and told her that she seemed to have lost her concentration and requested her to take a leave for two days to make up her mind. During those two days, she went home to her parents, only to find out that they were already separated. She accompanied her mother, who went on crying the whole day while she was there. Since she was the eldest, A.M. decided to stay with her mother for a week.
Additionally, A.M. explained that she felt guilty for not telling her family right away, thinking that she was at fault for her mother’s misery. During her stay with her mother, she tried to be strong in front of her during the day but cried during the night. Both A.M. and her mom lost appetite resulting in more weight loss during her stay.
A.M. explains that sometimes at work, she stares at nothingness, contemplating how incapable she is then just wanting to shoot herself. Currently, she had around 20 suicidal tendencies. The most recent was yesterday, where she tried to cut her left radial artery, which was why she was brought to the emergency room. It was the first time that she tried something to hurt herself. However, now, she stated that she wants to seek help and undergo psychiatric treatment in the hospital, where she feels safe. A.M. never suffered from paranoia, hallucinations, and feelings of grandiosity.
Past psychiatric history: A.M. got pregnant when she was 20 years old and gave birth to a baby boy, VI, who had a failure to thrive. Initially, A.M. was already frustrated because the child's father left her but decided to continue the pregnancy. After birth, she felt depressed seeing her child and frequently cried and lack the willingness to live. Eventually, she was diagnosed with post-partum depression.
