Part one: Clinical Experience
In the clinical area, every clinical encounter is quite challenging. The difficulty emerges not only with diagnosis and treatment but also the practice of entailing care that is safe, effective, efficient, accessible, and patient-centered. Again, managing a patient with more than one comorbidity is sophisticated because it requires a substantial competence to navigate the healthcare system, including various healthcare delivery points. For this case, I encountered a patient, X, with Type 2 diabetes (T2DM), hypertension, and hypothyroidism. This case typical case that healthcare providers often find difficult to manage due to varying patient factors and the need for higher competence from the healthcare provider to treat such cases. In most clinical situations, managing patients with a plethora of illnesses such as diabetes, hypertension, and deranged thyroid functioning require collaborated care, interprofessional partnerships, and coordination, and active participation of the patient in the overall treatment plan.
In light of this case, I firmly understand each disease, i.e., Type 2 diabetes (T2DM), hypertension, and hypothyroidism, in terms of pathogenesis, causes, risk factors, diagnosis, treatment, prognosis, and their interrelationships when they co-exist. There is an intricate relationship between Type 2 diabetes (DMT2), hypertension, and thyroid diseases. There is a postulated mechanism contributing to T2DM in thyroid dysfunction involving perturbed genetic expression combined with aberrations leading to impaired muscular glucose use, hepatic glucose output overproduction, and increased splanchnic glucose absorption (Wang,2013). Equally, the relationship between hypertension and diabetes, especially Type 2 diabetes (DMT2), is mostly discussed in clinical discourses, where these two diseases are considered results of metabolic derangements. Hypertension and diabetes share almost similar risk patterns.
