Even after a patient's death, hospitals keep their medical records. There are several reasons why medical records should be retained even after the patient has died. Besides this, in some states, keeping medical records of the deceased is required by law. Keeping medical records allows hospitals to keep accurate records, improve service quality, and communicate with the deceased's family. More importantly, it can aid in understanding the progression of a disease and its trends (Bayes, 2021). As a result, it is critical to keep patients' medical records even after they have died.
2 What procedure should a medical office assistant follow if a patient asks to read their chart? Why?
The medical office assistant cannot read the chart to the patient directly. The medical office assistant must request that the physician read the patient's chart. There are many medical terms in the chart that the assistant may not adequately explain, and there may be questions that only the physician can answer, so the physician should read the chart (Bayes, 2021).
3 What two legal documents served on a physician is necessary for patient records to be removed from an office?
Unless otherwise specified by law, a patient's record may not be deleted or removed from any institution. As a result, a physician will accept a court order or a subpoena if the patient requests that the records be removed (Bayes, 2021).
4 How should the transfer of medical records from one doctor to another doctor be handled? Explain steps taken.
Patients may request that their medical records be transferred. In other words, a medical record can be passed from one doctor to another. During this process, the patient will consult their new preferred doctor to determine the required information. The patient will then speak with his or her current doctor and request a transfer. After this, the patient has to wait for the transfer to be completed (Bayes, 2021).
5 Explain the responsibility of the physician when a patient or the patient’s authorized agent requests the issuance of a report with respect to an examination or treatment.
The physician must ensure that all information is kept private; therefore, the physician must disclose the information to the patient or the patient's authorized agent. In some cases, disclosing the information would adversely affect the patient's health. Thus, the physician must not disclose it (Bayes, 2021). As a result, the physician bears a great deal of responsibility.
