Questionnaire for Spiritual Assessment
1-Who or what provides you with strength and hope?
I gain strength from God. I also gain strength from my 4 children. I know what it means to them to have me in their life.
2-Do you use prayer in your life?
I pray every day. I pray for God to give me the strength I need to continue.
3-How would you describe your philosophy of life?
I feel a bit confused right now as to my philosophy of life. I believe things happen for a reason, whether or not we understand what that reasoning is. I believe in prayer but sometimes question whether our life is pre-determined,
4-What type of spiritual/religious support do you desire?
I desire inner peace and fortitude. I feel good when I attend church services even though I have fallen out of that practice for at least a few years. Being the turmoil my life has become recently, I have been craving visiting my church, even if it's just to be there by myself. It feels very peaceful when I'm there. It gives me strength to go on.
5-What is the name of your clergy, ministers, chaplains, pastor, rabbi?
I have attended Epiphany catholic church for many years. The monsignor is my favorite, Monsignor O'Daughtery. Very kind and inspiring man.
6-What does suffering mean to you?
Suffering means inner turmoil to me. Not being able to help those I love. Seeing people not be able to control what happens around t--hem, being in pain or not being able to provide our most basic needs.
7-What does dying mean to you?
I have no doubt our souls continue on. Our bodies are just a temporary vessel. I don't necessarily believe in "Hell". I believe there is an incredible experience waiting for all of us. Why would God make us suffer?
8-What are your spiritual goals?
My spiritual goals are to gain strength so I may continue to help those around me. I am very sensitive to my surroundings and people's feelings. I feel so overwhelmed sometimes when I can't change what is happening. I need strength and inner peace.
9-Is there a role of church/synagogue in your life?
I was brought up in the Catholic faith attending church services regularly. I continued that role with my children. In recent years I have fallen out of that routine. I now find myself wanting to return.
10-How do you keep going day after day?
Many days I don't even want to get out of bed. I think of my children and how I MUST push forward. For them and for me. I ask God and my mothers to help me. I ask for a lot of guidance. I need it. Also. I thank God almost daily for my job. Going to work gives me a purpose and is very important to me. My friends/co-workers give me strength and lot of love. I need and love them dearly.
11-What helps you get through this health care experience?
My faith that things happen for a reason, even though this is an incredibly hard time for me and my family. I know God will see us through this. I just wish it wasn't so hard. I know in the end, we will all grow from this experience. Seeing how all the kids are responding to their father's sickness is very heartwarming and rewarding. Part I: Interview
Select a patient, a family member, or a friend to interview. Be sure to focus on the interviewee's experience as a patient, regardless of whom you choose to interview.
Review The Joint Commission resource found in topic materials, which provides some guidelines for creating spiritual assessment tools for evaluating the spiritual needs of patients. Using this resource and any other guidelines/examples that you can find, create your own tool for assessing the spiritual needs of patients.
Your spiritual needs assessment survey must include a minimum of five questions that can be answered during the interview. During the interview, document the interviewee's responses.
The transcript should include the questions asked and the answers provided. Be sure to record the responses during the interview by taking detailed notes. Omit specific names and other personal information through which the interviewee can be determined.
Part II: Analysis
Write a 500-750 word analysis of your interview experience. Be sure to exclude specific names and other personal information from the interview. Instead, provide demographics such as sex, age, ethnicity, and religion. Include the following in your response:
1. What went well?
2. Were there any barriers or challenges that inhibited your ability to complete the assessment tool? How would you address these in the future or change your assessment to better address these challenges?
3. How can this tool assist you in providing appropriate interventions to meet the needs of your patient?
4. Did you discover that illness and stress amplified the spiritual concern and needs of your interviewee? Explain your answer with examples.
Submit both the transcript of the interview and the analysis of your results. This should be submitted as one document. The interview transcript does not figure into the word count.
