Networks address chronic illnesses by mobilizing diverse stakeholders to enhance service delivery efficacy at the local levels. Interorganizational networks recognize that no sector or organization is primarily responsible for the prevention of chronic illnesses. In this way, inter-organizational networks harness different individuals and agencies’ skills, capacities, and priorities to strengthen preventive efforts within the public health system and mitigate exposure to non-delivery risk (Hopkins et al., 2021). Networks provide continuity by cultivating robust patient-general practitioner engagements to enhance preventive care uptake and improve treatment adherence. In such contexts, general practitioners can monitor and follow up with patients with chronic illnesses such as diabetes to improve health outcomes. In addition, networks offer coordination between systems by encouraging a shared electronic health records framework to facilitate interprofessional and inter-organizational care planning for patients with chronic illnesses (Foo et al., 2021).
