Accountable Care Organizations (ACOs) are groups of health care organizations, physicians, and other medical providers organized voluntarily to be mutually tasked with managing the cost and quality of a targeted Medicare patient populace. ACOs are effective candidates for providing or making referrals to evidence-centered plans since they are designed to address both social and clinical determinants of medical services as part of the patient-centered planning care management framework (National Council on Aging, 2021). ACOs have transformed care delivery and reimbursement approaches to ensure patients receive higher quality, timely, well-coordinated, and patient-centered care. Patients within an ACO obtain care from healthcare facilities. Physicians are reimbursed for enhancing their patients’ well-being by delivering the proper care at the right place and at the right time (Wilson et al., 2020).
