Implementation Plan to Improve Diabetes Management Among African America Community in Maryland
Introduction
The increased cases of Type 2 Diabetes among African Americans living in Montgomery County, Maryland, have led to the identification of diabetics as a public health concern. The rising numbers of Type 2 diabetes among the targeted group have been associated with inadequate information on diabetes management. Poor diet and lack of physical health activities have also been termed as the possible causes of type 2 diabetes among African Americans in Maryland. This article is documentation of the implementation plan aiming to improve diabetes management knowledge among African Americans in the Maryland region.
Partnership and Support
To successfully disseminate information on diabetes management involves collaborations of various stakeholders who shall be working closely with the target community intending to create awareness. The collaborations shall therefore involve partnerships between public and private entities. The stakeholders from the public sectors shall include health workers such as nutritionists, who shall advise the public on a healthy diet. On the other hand, nurses shall aid in helping the public conduct several tests such as measuring their sugar levels, weight, and pressure. On the other hand, the private sector shall contribute to the course by providing physical trainers and even community health volunteers who will assist the nurses in teaching the public on diabetes management issues. Partners from the mainstream media and corporates will also be requested to support the course in several ways, including sponsoring informative adverts on various media platforms and financial support.

Besides the volunteering partners such as nurses, nutritionists, and other stakeholders from the private sector, the project might need to outsource to other professionals who will have to be paid for their services. This is likely to happen when volunteers in the project are overwhelmed by the anticipated massive number of African Americans targeted in the region.
