It is usually quite a difficult to have medical practitioners properly diagnose certain ailments such as Alzheimer’s. These are conditions that need thorough checking through brain and body scans to identify any protein that is associated with Alzheimer’s disease (AD) (Laureate Education, 2019). Other tests are geared towards assessing the patient’s cognitive functions such as memory, perception and language. Also, the patient’s age can be a good pointer towards the possibility of the patient suffering from AD, as it can start from 65 going forward. According to Rosenthal and Burchum (2021), there is also a genetic link that predisposes a patient to AD.
Summary
The current case presents a 76-year old male patient who has been brought in by one of his relatives. His son reports of the patient displaying certain unbecoming behavior, while his cognition has been deteriorating over the past few years. Judging on his age, the most likely test would be a memory test to ascertain whether he has any manifestations of AD. His mental orientation is quite irregular, and he therefore experiences phases of disorientation. His impulse control, insight as well as judgement also appear to be quite flawed. There is some evidence of confabulation and a short exam places the patient at a state of mild AD (Speed Pharmacology, 2019).
Decision 1
Administration of donepezil 5mg to the patient, who is supposed to keep taking it just before going to bed. The drug has been found to be quite helpful in compensating for the decreased functioning of cholinergic neurons by increasing the levels of acetylcholine in the brain (Linn et al, 2020). Previous research has revealed that donepezil has beneficial impacts on the cognition, impulse control and insight of an AD patient. This clearly illustrates that it is an important piece of medication that can be used in the management of AD. Its side effects includes weight loss and nausea in some patients (Rosenthal & Burchum, 2021). The other option can be the use of 4mg of galantamine administered orally.
