The health care sector in the US has been undergoing radical changes in the last few years, in a bid to revolutionize the way citizens access health care services. Different policies have been implemented in the last decade, which will foresee changes in health sector, going into the future. At the same time there have been changes in the health care delivery relative to the adoption of technology in the various levels(Thefutureofnursing.org, 2015). Of all the health care professionals, nurses are bound to be affected by the changes relative to their numbers. The key element for the nurses and the rest of the health care professionals is to adapt to the changes and formulate strategies to ensure that they take advantage of the changes to advance their career and most importantly to elevate health care delivery.
In the past, health care has involved the treatment of certain aspects that are ailing patients, whether they have come in as out-patients or they are admitted to the hospital. Once they recovered, they would be discharged and that would mark the end of that encounter with the patient(Himss.org, 2015). Concepts like continuum of care have come into play and revolutionized this aspect. This means that, nurses are faced with the need to treat patients and track their health care development over a long period of time. This also means that the patients have to be taken through a wide array of health care services other than what they have been treated for in the past(Himss.org, 2015). It is also important to note that this is not limited to patients who have been admitted or out-patients it also applies to those in nursing homes and those who are treated from their homes as well.
The aspect of continuous health care delivery can also be tied to that of medical homes. Ideally these health care models that bring together a team which is headed a physician(Pcpcc.org, 2015).
