Scenario:
Jax is a 66-year-old Caucasian female whose wife has encouraged her to seek treatment. She has never been in therapy before, and has no history of depression or anxiety. However, her alcohol use has recently been getting in the way of her marriage, and interfering with her newly-retired life. She describes drinking increasing amounts over the last year, currently consuming approximately a six-pack of beer per day. She notes that this amount “doesn’t give me the same buzz as it used to.” She denies ever experiencing “the shakes” or any other withdrawal symptoms if she skips a day of drinking.
Jax comments that her wife is her biggest motivation to decrease her alcohol use. She tells Jax that she gets argumentative and irritable when she drinks, though she does not always remember these incidents. She has also fallen while intoxicated twice, causing bruises both times and hitting her head on one of the occasions.

Based on the case presented, there are already issues with the alcohol consumption of Jax. This is already resulting in problems in her marriage and overall living condition. The problems demonstrated in the case involve the unhealthy and abnormal alcohol consumption of Jax, amounting to a six-pack of beers per day. She is also showing signs of not having the same effect upon consuming alcohol, leading to the need to consume more to get that buzz effect. This is a sign of alcohol tolerance and will only worsen if she continues chasing for the feeling of buzz and increasing the dose of alcohol intake. There are also behavioral changes whenever Jax is drunk, transforming her into a very irritable and argumentative person, affecting her relationship with her spouse. It is also worth noting that due to her alcohol use, Jax also forgets incidents such as her untoward behavior when interacting with her spouse. It is also definitely a problem because there is a contradiction happening. Even though her main motivation is her spouse, there are moments that it is the spouse who becomes the recipient of the untoward actions of Jax. Due to her intoxication, she is also becoming a threat to herself because of the increased alcohol in her system, leading to her inability to be conscious enough to function. The incidents of falls are already present, causing bruising and even leading to her injuring her head. She also demonstrates a level of intoxication where she already has poor equilibrium, memory loss, and blackouts. In making a diagnosis, it is also important to note that the patient/client denies withdrawal symptoms and that she can stop or skip drinking alcohol for a day without any adverse effects. Although, this statement should be confirmed by a reliable source such as her spouse to ensure the credibility of the statement, as there are a lot of factors that may affect the response of a patient/client.
For additional information, inquiries should be made regarding whether there was already an attempt to address the level of her alcohol consumption. If so, the specific methods used and her body and mind's response to it should also be asked. The presence of craving, including its severity and frequency, is also important for evaluating and treating the patient/client (McHugh & Weiss, 2019). Even though Jax is already retired, other roles and obligations in society affected by her alcohol consumption should be asked. Lastly, the quality of sleep that Jax experiences for the past months, especially during worsening her habits, should be considered part of her evaluation and treatment process.
